Job Description
Position Summary: Under the direction of supervising registered nurse, the LVN will provide patient care in the acute care setting of the urgent care (UC) clinic, in accordance to delegated duties and scope of practice.

Essential Functions of the Job:
- Upholds San Ysidro Health’s (SYH) Policies and Procedures and Core Values
- Performs patient care functions as prescribed by providers, following established UC protocols and procedures, and within defined scope of practice
- Gathers patient’s data collection focusing on physiologic, psychosocial, cultural, spiritual, and cognitive status
- Perform nursing tasks including but not limited to: 12-lead EKGs, Intravenous (IV) access insertion, administer PO, IM/SQ and topical medications as per provider’s order, urinary catheter insertion, respiratory treatments, and provide patient/family education upon disposition
- Perform POC testing and assist providers with therapeutic and diagnostic functions
- Displays understanding of standard pharmacological agents used in treating patient conditions
- Demonstrate principles of aseptic techniques and infection control and prevention measures, as per SYH guidelines
- Document pertinent patient information (i.e. completed orders,nursing procedures, patient’s response to treatment, etc.) in the EHR, as established by SYH guidelines
- Monitor patient’s response to treatment and communicate with other health team members significant changes and modify care plan accordingly
- Demonstrate understanding of policies, protocols, organizational structure, and appropriately utilize chain of command
- Participates regularly in staff development trainings to ensure continuous quality improvements
- Demonstrate and apply knowledge of safety principles as identified by SYH
- Follow National Patient Safety Goals and Core Measure Initiatives
- Maintain department readiness at all times (i.e. clean/organized work area, restocking, equipment checks, and exam room preparation)
- Ensures clinical area is audit prepared by completing monthly Quality Assurance packets
- Participates in department performance and Quality Improvement activities
- Provide assistance with administrative task (i.e. nursing schedule, supply orders, etc.) as needed or assigned by management
Education Required
(Minimum level of education):
- High School Diploma or GED Equivalent
- Graduate of an accredited Vocational Nursing Program
- Current California LVN License
- Basic Life Support (BLS) Certification (American Heart Association)
- Minimum of six (6) months of experience in urgent care setting (within past 3 years)
- One (1) – Three (3) years of experience in acute care setting.
- Associate of Science Degree (AS)
- Effective interpersonal and communication skills.
- Knowledge of legal implications in clinical practice and documentation
- Demonstrates accountability and professionalism
- Basic computer skills and experience or familiarity with HER
