JOB TITLE: Unit Clerk
DEPARTMENT: Long Term Care Center
REPORTS TO: Director of Nursing of LTCC
JOB SUMMARY: Coordinates the various activities on the unit, i.e., greeting and directing visitors, residents, physicians, staff members, etc. Responsibilities include assisting with new admissions and discharges/transfers; insuring information in resident charts is accurate and up-to-date. Assists nursing staff with various non-clinical duties, such as managing equipment for residents as well as supplies and equipment for the unit.
ESSENTIAL FUNCTIONS
- Greet residents and visitors at the desk in a friendly, courteous manner. Present a positive image at all times while directing residents, visitors and staff to appropriate areas. Make all residents and visitors feel welcome and cared for. Enforce visiting policies as requested by nurse.
- Coordinate admission of residents. Complete proper admission documents when LTCC business office is closed (resident information sheet, etc.). Assist nurse with room assignments when necessary. Initiate labeled medical record on admission. Assure prompt completion of admission diagnostic work with assistance from RN/LPN. Notify nurse of any stat treatments or medication.
- Coordinate discharge/transfer of resident. Prepare medical record for discharge. Take completed medical record to LTCC business office after discharge of resident.
- Answer telephone efficiently and courteously; relay messages and calls to appropriate personnel and residents; relay information to other departments as needed. Answer resident intercom promptly and courteously; refer urgent requests to the nurse immediately.
- Transcribe physician orders. Make requisitions for diagnostic studies, make out Medication Administration Record (MAR) and Kardex; assure completion of stat orders. Notify the nurse of new orders and assure orders are double-checked for transcription by the nurse. Refer telephone and verbal orders to RN/LPN. Note initials on orders transcribed.
- Coordinate activities of nursing with other departments. Note departure and return of residents to and from other departments and out of facility. Notify other departments of pertinent changes in status or physician orders. Assist nursing staff with obtaining needed equipment or supplies from Central Supply. Will do PAR ordering.
- Maintain medical records in an orderly manner. Keep charts supplied with needed forms. Make sure all forms are stamped with ID plate. Obtain past medical records form Health Information Department under direction of the nurse. Operate copy machine.
- Assure recording of patient information in medical record. Post lab slips and x-ray results. Notify charge nurse when information is missing. Charting weight from flow book to chart.
- Make appointments for residents, outside medical and dental visits and provides transportation to the appointments by facility bus.
- Assist Director of Nursing with data collection for Quality Improvement (QI) studies.
- Monitor residents’ charts for annual History & Physicals.
- Keep desk stocked with needed clerical supplies.
- Notify appropriate departments of maintenance alarms. Complete work orders.
- Participate in monthly staff meetings, in-services when able and committee assignments intended to improve resident care.
- Answers resident call lights when necessary and assists residents as appropriate.
- May be required to perform other duties as qualified.
JOB REQUIREMENTS: Current Wyoming Certified Nursing Assistant certification. A temporary nursing assistant certification may be accepted upon hire, but the permanent certification must be obtained prior to the expiration date of the temporary license.
EXPERIENCE: Training in secretarial skills and medical terminology essential. Evidence of skill in written and verbal communication very important.
SKILLS: Must be an individual who enjoys working in a busy, crowded, fast-paced setting. Good verbal and written communication skills are essential. Must be able to maintain a high degree of accuracy under stress, complete monotonous detail work without error and handle several tasks at once. Must be willing and able to learn terminology and equipment operation. Good penmanship necessary. Maintains confidentiality.
REQUIRED CERTIFICATIONS: C.N.A., BLS and a current Wyoming Drivers’ license
