Transitional Care Manager

Piper Clinical is currently seeking a Transitional Care Manager in the area of New Orleans, LA. The Transitional Care Manager will be responsible for supporting a leading integrated care provider through the coordination of care and case management of at-risk members. This opportunity also includes up to a $5,000 sign-on bonus, based on relevant experience.

Responsibilities for the Transitional Care Manager:

• 
  Coordinate care plans for at-risk members during admission and follow-through up until discharge from the in-patient facility (Up to 30 days)
• Ensure members on assigned cases are following their case plans accordingly
• Support members by navigating them through their care plan to improve quality of life and health outcomes in order to decrease unnecessary emergency room visits

Qualifications for the Transitional Care Manager:

• 
  At least 2 (two) years of Case Management experience with at least 2 (two) years of RN experience
• Active RN License in Louisiana
• Bachelor's Degree with CCM or CCTM certification preferred

Compensation for the Transitional Care Manager:

• 
  $90,000 - $100,000 (Based on relevant experience)
• Remote Flexibility
• Up to a $5,000 sign-on bonus, based on relevant experience
