Transitional Care Associate

Position Summary:
Supports the functions of the Transitional Care department through the education of patients and families regarding required regulatory notices, assists with patient discharges arrangements, and provides specialized administrative assistance, record keeping and general care-coordination tasks as assigned by the Care Manager and Medical Social Work partners.

Responsibilities:
Performs other duties as assigned to support department needs
Familiar with standard concepts, practices, and procedures within the field
Relies on experience and judgment to plan and accomplish goals
Regular and predictable attendance is an essential job function
Competent to meet age specific needs of the unit assigned

Requirements and Minimum Qualifications:
Associate’s degree required, Bachelor’s preferred
Prior clinic, hospital or health related field experience
Minimum of one year of clerical experience in a health care setting or social services agency
Knowledge of and ability to communicate with community resources: Home health, durable medical equipment, oxygen, skilled nursing facilities and transportation companies (information, referral, and prior authorizations)
Knowledge of insurance benefits, Medicare, Medicaid, commercial plans and medical terminology

Working Conditions:
Must be able to maintain a standing position
