This position has 2 main focus areas:
(1) To provide person-centered support in a team environment to TAY clients in the Residential Rehabilitation Program (RRP). The focus is on teaching skills both individually and in groups and accessing resources for resiliency, recovery and independent living and success in life. It includes assessing the young adults’ needs and strengths, assisting with development of the rehabilitation plan, assessing progress, and maintaining the clients’ medical record. Also included is providing needed transportation and meeting productivity measures and daily documentation requirements.
(2) To provide data entry and analysis support to the TAY program.
ESSENTIAL DUTIES:
Assist clients in assessing their strengths, needs, resources and desires and in developing their Individual Rehabilitation Plan (IRP) as frequently as required or needed.
Work with RRP Team in coordinating services and accessing resources needed to promote recovery and resiliency and life skills necessary for independent living and success in life.
Assist clients in meeting their goals by working with them in the residence, at Club or in the community across all program sites.
Develop group curriculums using IMR and other resources as assigned.
Meet agency productivity measures specific to the position.
Utilize computerized database(s) or other procedures as required to submit accurate, timely and professional documentation of services in accordance with agency policies, regulations and accreditation standards.
Provide transportation to clients when needed.
Assume responsibility for a healthy and safe environment.
Assist with meeting goals for program effectiveness, efficiency, consumer satisfaction and accessibility.
Assist the agency to comply with agency policies and procedures, legal, regulatory and accreditation standards.
. Requirements:
EDUCATION & EXPERIENCE REQUIREMENTS: Highschool Degree is required and at least two years positively referenced work history in human services field. Experience working with transitional aged youth is preferred.
SCHEDULE: Flexible to meet client needs. May include evenings and weekends.
