CASES is a New York City agency working at the forefront of the intersection between behavioral health and involvement in the criminal justice system. The Behavioral Health division consists of a continuum of services for justice-involved individuals with serious mental illness, including outpatient clinic, health home care management (HHCM), assertive community treatment (ACT), and intensive mobile treatment (IMT).

CASES’ Health Home Plus (HH+) Care Management Program, located in Central Harlem, is an intensive Medicaid service for adults diagnosed with a serious mental illness (SMI) who may benefit from enhanced support because of high needs, disengagement from care, and/or poor outcomes (e.g., multiple hospitalizations, incarceration, and homelessness). HH+ care managers work directly with individuals in the community to help them establish and maintain participation in the services they need to address their treatment, social, and health needs. Care managers attend appointments with participants and help them to acquire skills that promote successful community living. The length of HH+ participation is determined by ongoing assessment and review of individual progress in the community. CASES’ HH+ Care Management promotes successful transition to the community and improved behavioral health and wellness outcomes by helping participants to:

  • Learn to manage challenges related to mental and physical illness
  • Reduce visits to emergency rooms and inpatient hospitalizations
  • Reduce risky behaviors including substance use that can lead to involvement in the criminal justice system
  • Achieve stability and safety as fully integrated members of the community

We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This job requires work in our offices and/or the community.

Job Description
  • Provide training, clinical supervision, evaluation, and professional development opportunities to staff members.
  • Provide clinical oversight to the team, including person-centered treatment planning, evidence-based practices, use of service dollars, high risk/crisis incident management.
  • Ensure program operates with high fidelity in accordance with the HHCM Scope of Services as per DOH/OMH.
  • Develop and monitor the program budget to maintain fiscal viability of the program.
  • Work with external stakeholders – such as hospitals, shelters, jails, service providers – to achieve positive client outcomes.
  • Provide services in a strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner.
  • Coordinates and oversees the services among care managers as well as the implementation of the care plan.
  • Team Leader maintains client-related records and other requirement documentation as well as conduct quality assurance activities on a regular basis.
  • Serve as a task manager for interns
  • Other duties as required by agency executive staff.

Qualifications
  • Active New York State license as Licensed Clinical Social Worker (LMSW), Licensed Mental Health Counselor (LMHC)
  • At least two years of supervisory experience in a behavioral health setting
  • At least two years of experience working with adults who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal-justice involvement.
  • Previous use of an electronic health record, Footholds, PSYCHES, MAPP, HCS, HARP preferred
  • Experience in a multidisciplinary setting and field-based clinical work preferred.
  • Fluency in Spanish a plus
  • Strong commitment to social justice and the CASES mission
