Job Description

The New York City Department of Health and Mental Hygiene (DOHMH) is expanding home visiting in NYC and streamlining how providers, organizations, and community residents can access quality home-visiting services. The goal is to improve maternal and child health and well-being by matching families' assessed risks to evidence-based (EB) or evidence-generating (EG) home visiting (HV) models that are most appropriate for them based on their needs, and to connect them to other resources as needed. Racial, ethnic, and socioeconomic inequities create disparate outcomes in certain neighborhoods as compared with the rest of the city, and providing services beginning in the prenatal through postpartum period is vital to improving these outcomes. In addition, recent attention to inequities in maternal morbidity and mortality, especially in New York City, points to the need for more respectful maternity care and greater agency for people during childbirth, both of which the program's staff will facilitate, in partnership with NYC doula organizations. DOHMH's New Family Home Visits (NFHV) program will serve families from pregnancy through the postpartum period. A subset of families will also be offered labor and delivery support by a certified doula. NFHV incorporates a strong mental-health and chronic-disease focus, including screening for diabetes, hypertension, maternal depression, and anxiety, along with referrals to needed services. The citywide program will begin in Brooklyn, offering home visits to more than 2,900 first-time expectant parents. of those. The neighborhoods burdened by adverse maternal and child health outcomes will be prioritized; however, all first-time expectant parents are eligible. The Administrative Public Health Nurse will be responsible for supervising the nursing staff who provide home visits to families with newborns. The APHN will direct the team of Family Public Health Nurses in order to schedule, visit, and support families in the community using the NFHV protocol. We are seeking a candidate who can demonstrate leadership and is able to inspire, influence, and enable others to achieve a specific mission and meet program goals. The Bureau of Maternal, Infant and Reproductive Health (BMIRH) is dedicated to improving and reducing inequities in maternal, infant and reproductive health outcomes through program, policy and research initiatives. The Bureau strives to apply a sexual and reproductive justice and racial justice framework to our work, and to fully engage community partners to move this work forward.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

Become fully trained in the NFHV home visiting program and support the citywide implementation of the program including recruitment staff training and protocol development.
Assist in training and evaluating new staff in the NFHV program, eventually being responsible for certification and fidelity of all staff to the protocol.
Track visiting rates and nurses' fidelity to the home visiting protocol through the use of computer-generated reports and dyadic supervisory home visits.
Provide group/peer as well as reflective supervision to nursing staff.
Work with team to ensure appropriate referrals and connections to community resources as needed. Identify gaps in community services that may be addressed by local partners.
Communicate regularly with local stakeholders with updates on the progress of the program.
Coordinate assignments to the nurses to provide home visiting services for eligible families referred by program staff
Coordinate staff meetings for case review and in-service training.
Supervise nursing staff and conduct quality assurance visits with supervised staff.
Collaborate with members from the program team to identify, engage and establish relationships with referral partners and other agencies/organizations providing services to pregnant and parenting families
Oversee the PS and OTPS line items related to nursing needs
Be open and flexible in establishing a new program with many unknowns in a new community with infrastructure challenges. Minimum Qual Requirements1. A valid New York state license as a Registered Nurse, a baccalaureate degree in Nursing from an accredited college and a master's degree in Nursing, Public Health or a related field, plus four years of recent experience in public health or hospital nursing or a related field; at least 18 months of which must have been in an administrative, managerial, executive, consultative or educational capacity, or in supervision of professional public health nursing personnel working in any of the above mentioned areas; or

2. Education and/or experience equivalent to "1" above. However, all candidates must possess a valid New York State license as a Registered Nurse, and a baccalaureate degree in Nursing from an accredited college; and must possess the 18 months of specialized experience as described in "1" above. Preferred SkillsAbility to communicate effectively with others is essential

Experience in maternal/child health, home visiting, and/or lactation education preferred
