Due to Covid-19 this position is currently remote, providing services via Telehealth. However, only local candidates will be considered as services are only expected to remain virtual during Covid-19.
Our Case Managers offers supportive services to clients to increase self-efficacy, self-advocacy, basic life skills, coping strategies, self-management of biopsychosocial needs, benefits and resources, and reintegration into the community.
Case Management works with clients from intake to discharge, while monitoring client progress towards thier treatment goals & objectives, and work closely in collaboration with other members of the multidisciplinary team to ensure best possible treatment outcome for the client.
This role requires attention to detail, strong computer skills, experience working with an EHR (Kipu preferred). The right candidate will be able to meet heavy documentation requirements, meet deadlines, and posses strong computer skills.
Job Duties:
- Work in collaboration with the client, their clinical team, and management to help clients execute their treatment plan goals & objectives
- Facilitate psycho-educational groups based on assigned content, and complete group documentation in a timely manner
- Research content for assigned groups, work with the clinical management team to develop a practical curriculum that addresses the needs of this specific population
- Provide support and care by linking client with appropriate services to their address specific needs such as: benefits, mental health, physical health, employment, probation, DCFS, employment, housing, community resources, and transition through the continuum of care.
- Initiate attainable S.M.A.R.T. goals for clients to complete the needed requirements to advance their level of care
- Document progress, advancement, and completion of changes in status regarding client.
- Complete Documentation within a timely and professional manner following the format assigned by clinical management
- Work with various departments to facilitate smooth transitions to meet clients care and needs
- Schedule medical and / or psychiatric appointments as needed for the clients
- Assures proper consents are signed and on file
- Initiate contact with the client’s family members weekly (based on consent) and assist therapists with family programming and interventions
- Maintains daily contact with client
- Meet the State of CA requirements as a mandatory reporter of specific crimes
- Corresponds with client’s probation, parole or social service agents as approved by management
- Maintains strict client confidentiality and adheres to all HIPAA regulations
- Adheres to all federal, state, county and agency regulations
- Performs crisis intervention for all clients in crisis, not only specifically assigned clients
- Educate Clients on life skills, personal advancement and educate client on resources available for them when then reintegrate back into the community for long term care of their holistic healthcare needs
- Completes CEU’s (continuing education units) pertinent to their line of work
- Work with Clinical director and Program Director to advance curriculum within the program through research
- Can be on call as needed for clinical and administrative support
- Other duties as assigned by CPBH upper management team
Minimum Qualifications :
- Current certification through CAADAC/CAADE as a Certified Alcohol and Drug Counselor or in final stages of completion (preferred)
- One year of experience as a case manager or social worker with strong professional references attesting to a high degree of clinical sophistication
- Advanced knowledge of evidence based practices and their application in residential and outpatient treatment
- Specific knowledge helpful in process addiction and treatment resources
- Advanced assessment, treatment planning, discharge planning and documentation skills
- Strong Microsoft Office Word, Excel, PowerPoint, and Google Drive
- Typing at 60 wpm or greater with typing test results upon request
- Working knowledge of current DSM-V diagnosis and definitions
- Knowledge of dual diagnosis treatment practices including non-pharmaceutical symptom management techniques
- Strong written and oral communication skills
- Ability to work with within a diverse team of colleagues and clients in a fast-paced environment
- Consistently able to meet deadlines assigned by management to complete projects
- Current certification in First Aid/CPR
- Ability to complete a Live Scan to be cleared with County and State of CA programs
- Valid CA driver’s license
