Job Description
Responsibilities: Creates optimal outcomes for patients/families by managing complex psychosocial and economic co-morbidities. Works in collaboration with case managers to insure timely and appropriate discharges. Through advance practice skills, mobilizes internal resources to reduce risks and assists patients/families in leveraging external resources.Qualifications:Reports:RN Manager/Case Management
Supervises:None
Required Education:Minimum: Master's Degree in Social Work (MSW)
Required Experience:Preferred: 1 years in social work or human services field, preferably in acute care setting.
