Job Description

Minimum QualificationsMaster’s degree in social work from an accredited institution as approved and accepted by the University of South Alabama; or Bachelors degree in social work from an accredited institution as approved and accepted by the University of South Alabama and two years social work experience; or Bachelor’s degree in a related field a from an accredited institution as approved and accepted by the University of South Alabama and three years social work experience. Licensure with the State of Alabama as a Social Worker.

Essential FunctionsAbides by and enforces all compliance requirements and policies and performs these responsibilities in an ethical manner consistent with the organization’s values; adheres to hospital policies including confidentiality; enhances professional growth and development through participation in education programs; requires minimal supervision to safely perform essential functions; utilizes proper body mechanics when: moving equipment/supplies that is necessary to perform essential functions; completes initial and ongoing assessments of patient and patient’s support systems, the response to diagnosis, living environment, and discharge planning needs; documents assessment findings and interventions in medical record per department policy; communicates and collaborates with patients, their families, physicians, therapy services, nursing, utilization review nurses and other departments as necessary to formulate an effective discharge plan; actively coordinates patient and family interventions with the physician, patient, and other members of the healthcare team while developing treatments plans with mutually agreed upon goals set forth by the patient and physician; evaluates the effectiveness of interventions and actions in relation to the intended goals of the discharge plan and revises intervention strategies as needed; identifies and documents barriers in service delivery system and advocates for change in processes as needed; maintains current knowledge and is familiar with federal and state regulatory guidelines, institutional policies and payer source policies; works closely with the RTN Discharge Planners, Utilization Management Staff and other healthcare team members to coordinate and implement a safe and appropriate discharge plan; attends and participates in interdisciplinary patient care rounds or conferences; assists outpatients with crisis intervention and referral to community resources as needed; supports families and patients dealing with ethical issues, disability and terminal illness; ensures continuity of patient care by acting as a resource for patient information; provides after hours on-call services on a rotating basis; communicates and uses appropriate customer relation skills with physicians, patients, families and healthcare team in person and via telephone; maintains accurate and complete records by documenting assessments and plans in patient’s medical record as dictated by department policy; provides employee counseling as requested; completes all mandatory unit, educational and hospital requirements; utilizes cost effective practices in performing all aspects of the job; adheres to current Infection Control and Safety Standards; demonstrates the ability to provide competent patient care to the following patient populations: Neonate, Pediatric, Adolescent, Adults and Geriatrics; participates on committees as assigned; participates in performance improvement activities as assigned; regular and prompt attendance; ability to work schedule as defined and additional hours as required; related duties as required.
