Job Description

Hennepin Healthcare is an integrated system of care that includes HCMC, a nationally recognized Level I Adult and Pediatric Trauma Center and acute care hospital. SUMMARY

We are currently seeking a Social Work Supervisor to join our Ambulatory Care Management team. This fulltime role will work days. Specifically seeking candidates with 2-5 years’ experience as team lead/supervisor and 3-5 years’ experience in the ambulatory clinic setting and/or community setting.

This Ambulatory Social Work Supervisor will provide direct supervision and support for Social Workers in ambulatory primary care and specialty care clinics at Hennepin Healthcare. Primary job functions include: 1) Provide consultation and advising to the outpatient social workers in patient cases with ethical, legal, or clinical complexities, 2) Identify and educate the outpatient social work team on relevant legislative or state statute policies, procedures, or protocol updates that impact their work with HHS patients, 3) Manage department staffing and provide coverage as needed, to maintain minimum level of social work support to outpatient clinics. LICSW required.

RESPONSIBILITIES

  • Participate in determining program goals and objectives and in preparing budgets and proposals
  • Develop policies and procedures for service delivery
  • Serve as liaison with other community agencies and other department programs
  • Supervise the internal social service reporting system within the unit
  • Administer and enforce the employer/employee labor agreement and personnel rules
  • Select and develop staff, plan and assign work, and evaluate employee performance
  • Consult on difficult case problems
  • May conduct studies of and provide social service for selected cases and programs
  • May be involved in integrated, multidisciplinary or community initiatives and may be housed at community based sites including serving as a liaison between Hennepin County and communities, community organizations, schools, health professionals and service providers and identifying needs and assist in resolving problems facing individuals and families regarding health and human service issues (including but not limited to housing, employment, financial assistance, and behavioral health)

QUALIFICATIONS

Minimum Education/ Work Experience:

  • Master s degree in social work or closely related field
  • Three (3) years of experience as a social worker

OR

  • Bachelor's degree in social work, psychology, sociology, or human services, plus 27 quarter credits of graduate work in areas such as social work methods, human growth, and behavior, principles of social welfare
  • Four (4) years of supervised experience as a social worker

OR

  • An approved equivalent combination of education and experience

Leadership/Knowledge/ Skills/ Abilities:

  • Skilled in overseeing the efforts of individuals within the department
  • Ability to effectively involve department staff in the application of quality measurement and improvement tools
  • Skilled in fostering a positive workplace culture and building inclusive workplace teams
  • Demonstrated ability to quickly learn new technologies and systems and provide instruction on complex processes
  • Capable of assisting managers with decisions and activities
  • Effective use of all Windows-based applications such as Word, Excel and Power Point
  • Ability to appropriately prioritize and carry-out work
  • Ability to comfortably and effectively facilitate meetings and feedback sessions

License/Certifications:

  • Some positions require eligibility for or licensure as an Independent Clinical Social Worker by the Minnesota Board of Social Work
  • Licensed as a graduate social worker by the Minnesota Board of Social Work
