Job Description

JOB SUMMARY:

Directs and supervises social service program. Evaluates the social needs of each patient admitted and formulates a written plan of care. Reviews and revises plan as necessary, as but not less often as quarterly. Refers patients and families to appropriate community resources as necessary. Responsible to the Administrator.


EDUCATION:

A college degree is mandated in Sociology or another related field approved by State Representatives.


QUALIFICATIONS:

  • Must have a genuine interest in the elderly.
  • Must exercise patience and tactfulness.
  • Possess good listening skills.
  • Must be able to execute a social service program appropriate to the needs and interests of the patients.
  • Must have knowledge of special needs and limitations of elderly.
  • Is able to recognize and identify situations in which problems are such that referral is indicated.
  • Has sufficient understanding and self-discipline to relate to the patients’ needs and feelings.
  • Has an interest in learning and advancement.
  • Possess the ability to work congenially with others.
  • Must be able to work hours as assigned by the Administrator.
  • Must have a pre-employment and an annual tuberculosis screening examination.

RESPONSIBILITIES:

  • Plans and organizes a social service program according to established policies.
  • Assists in the coordination of resident transfers.
  • Handles telephone calls and interviews each patient and family on admission.
  • Coordinates services with other departments.
  • Completes a Social History on each patient.
  • Identifies and evaluates personal, emotional and environmental concerns.
  • Formulates a written plan of care for each admitted patient.
  • Reviews and updates plan of care according to established policies.
  • Records continuing progress notes on the patient's social needs and response of the patient to delivered social services.
  • Is responsible for the Discharge Plan.
  • Assesses spiritual needs and facilitates participation in religious activities.
  • Participates in the development and review of the Total Care Plan.
  • Assists with the development of a community volunteer program.
  • Conducts annual in-service on social services for all staff.
  • Assists in coordinating the Resident Council.
  • Helps open and read incoming mail, and helps address and post outgoing mail upon the patient's request.
  • Assembles and maintains a store of information on community resources and helps other staff to use these accordingly.
  • Gives assistance or help in locating and arranging for services of other professionals or agencies to solve problems and needs of patients.
  • Participates regularly in case conferences with nurses and other staff to assist in better understanding the patient and his family.
  • Helps needy patients and families locate funds for dentures, glasses, etc.
  • Participates in pre-admission screening of prospective patients whenever possible.
  • Conducts facility tour and orientation for new patient and family. 23. Listens to the residents. 24. Performs other various duties as deemed necessary by the Administrator.

PHYSICAL DEMANDS: Use the percentages range as follows:

Requires full range of body motion including: 1. Standing/Walking: Frequently.
  • Worker will be spending considerable time up and about in the facility doing various tasks as per job description.
2. Bending/Stooping: Frequently.
  • Worker will be bending or stooping to work with supplies, preparation of documents, filing, working with clinical records, etc.
3. Lifting/Handling: Occasionally.
  • Worker will be lifting/handling supplies to do inventory, restocking, ordering, etc. as well as filing and auditing of clinical records. Range of weight: 1 - 50 pounds.
4. Carrying: Occasionally.
  • Worker will be carrying clinical records, manuals, supplies, etc. Range of weight: 1-50 pounds.
5. Pushing/Pulling: Occasionally.
  • Worker will be pushing/pulling on various supplies storing, moving and inventory of supplies storing, moving and inventory of supplies, as well as with clinical records filing, auditing, preparing for each month, etc. Range of weight: 1 -50 pounds.
6. Balancing: Occasionally.
  • Worker will be balancing supplies to be stored, moved from storage to using area, etc. When moving records from active to inactive status or during auditing periods worker will be balancing legal documents that are not secured.
7. Pivoting/Turning: Frequently.
  • Worker will be pivoting and turning stock supplies, handle records, etc.
8. Crouching/Stooping: Frequently.
  • Worker will be crouching/stooping to file records, working with supplies to restock inventory of supplies, etc.
9. Kneeling: Occasionally.
  • Worker will be kneeling to file records, stock and inventory supplies, etc.
10. Reaching:
  • Worker will be reaching to file records, stock and inventory supplies, etc.
  • A. Greater than shoulder height: Occasionally.
  • B. Equal to shoulder height: Occasionally
  • C. Less than shoulder height: Occasionally
11. Handling/Manual Dexterity: Continuous.
  • Worker will be using fine and gross motor dexterity skills in all of job requirements.
12. Speaking/Hearing/ Seeing: Continuous.
  • Worker must be able to communicate with co-workers, residents, families, etc. face to face as well as telephone communication.

JOB LOCATION:

Must work throughout all nursing areas of the facility. May require working irregular hours and may be exposed to communicable diseases and/or body fluids, medical preparations, toxic substances and occasionally ionizing radiation.
