JOB
The City of Norfolk is seeking a Service Coordinator/Case Manager III to join the Infant and Toddler Connection team within the Norfolk Community Services Board. Provides Service Coordination/case management services to children, birth to three years of age, with developmental delays, diagnosed conditions or atypical development & their families by assessing, planning, linking, monitoring, and advocating for them in response to their changing needs. Completes clinical documentation in accordance with agency and regulatory guidelines.The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.Today, the CSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. We are also seeking persons to join our team that are dedicated to supporting a positive work and treatment environment. EXAMPLE OF DUTIES
Essential functions include but are not limited to: Provides case management and related services for specified client populations in various human service areas.Completes initial and periodic assessments of specified groups for planning, needs areas, and eligibility for services.Develops service plans based on assessments and in collaboration with individuals and/or families.Provides client linkage to, coordination, and monitoring of services; identifies and accesses resources to meet needs and assists clients in procuring and maintaining resources and entitlements; may also provide client advocacy to provide needed resources or services, while empowering them to be as independent as possible.Follows up at specified intervals to assess ongoing eligibility for services, progress toward meeting service plan outcomes, and client and/or family satisfaction.Prepares and maintains documentation in compliance with agency, state, and federal guidelines and regulations; complies with privacy and reporting requirements.Complies with applicable local, state, and federal regulations for provision of information and reporting abuse and policy violations.Performs related work as assigned.
MINIMUM QUALIFICATIONS
Bachelor's degree in a human services field.Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
