Position Purpose
Service Coordinators plan, coordinate, and monitor services provided to individuals with intellectual or developmental disabilities and their teams. Individuals served may receive support through a variety of waivers, state, or local funding including: HCBS-DD, HCBS-SLS, HCBS-CES, State SLS, and Denver Mill Levy.
The Project Service Coordinator needs to have the basic knowledge and experience on how to perform essential Service Coordinator duties (below) if the need to float/assist with cases may arise. This individual also needs a broad working knowledge of the I/DD system as it relates to assisting or leading specialized projects related to CCB functions such as the Human Rights Committee and other evolving systemic functions. This position may work closely with the SC management team on various projects as needed in a flexible, autonomous fashion that will further promote further efficiency and quality customer experiences.
Essential Duties:
- Responsible for a small caseload of customers receiving State programs and/or HCBS waiver services
- Provides Targeted Case Management functions, including billing through contact notes
- Facilitates, writes, and monitors annual service plans for individuals with intellectual or developmental disabilities
- Initiates and writes referrals for services
- Aids with service and support placements
- Assists in transfers and placements to new waiver programs as needed
- Communicates effectively with clients, families, agency staff and others to obtain positive outcomes
- Reviews, responds, and provides documented follow up regarding any client paperwork
- Provides information and knowledge about the waiver programs to the client and their team
- Completes face-to-face visits with individuals in the community and at their homes quarterly
Serves as a mediator and helps to resolve conflict as it relates to the individual’s service plan
Advocates for the best interest of the client while valuing person centered approaches of empowerment
- Conveys a positive and professional image of the organization to others
- Performs other duties as assigned
Additional Project Service Coordinator Duties:
- Organizes, documents, and tracks functions related to Human Rights Committee/Modification of Rights
- Facilitates and documents recommendations at monthly Human Rights Committee meetings as needed
- Communicates accurate information to PASAs and other stakeholders as needed when assisting with questions for projects and HRC
- Communicates effectively with SC management team regarding project progress, needs, and outcomes
- Effectively manages time by planning and proactively working toward project goals in an efficient and professional manner
- Other duties as assigned
Qualifications
Knowledge, Skills and Abilities
- Ability to effectively communicate both written and verbally
- Knowledge and understanding of people with developmental disabilities, HCBS waivers, and state/local funding sources
- Ability to maintain strict confidentiality
- Ability to critically analyze information and use sound judgement when making recommendations or solving problems
- Ability to effectively manage and organize multiple priorities and tasks
- Knowledge of basic financial management skills
- Basic skill in computer software programs
- Ability to function as a cooperative team-player along with working autonomously when needed
- Skill in mediation and conflict-management
- Knowledge of community resources
- Excellent facilitation skills
- Excellent interpersonal and problem-solving skills
Essential Functions

Minimum Qualifications
- Bachelor’s degree in Human Services (or related field) or five years working with people with developmental disabilities and their families or a combination of education and experience.
- Must have a valid driver’s license, proof of insurance, reliable transportation, and a proven ability to safely drive personal vehicle without exposing company to serious liability risks
- Ability to receive, process, and maintain information through oral and/or written communication effectively
Preferred Qualifications
- Fluent in written and spoken Spanish
- Bachelor’s degree and two years working with people with developmental disabilities and their families
Driving Requirements
- Valid drivers license
- Proof of motor vehicle insurance
- Personal vehicle in good operating condition for use during work
- No major violations in the past three years.
- No more than two moving violations in the past three years
- Ability to meet and maintain agency driving requirements and operate agency vehicles
