POSITION OVERVIEW
Consumer Direct Care Network is seeking a self-starting, energetic and people-oriented Service Coordinator (RN and LPN) to work in a dynamic and fast paced environment located in our Great Falls office. In this important role you will provide training and support to our members who are self-directing their services as well as managing staff that support members with traditional model of services. The Service Coordinator will be responsible for arranging and conducting in-home visits, and are involved in day-to-day operations, referrals, intakes, and program monitoring. Successful candidates will possess strong interpersonal abilities, provide exceptional customer service, and has a high value for teamwork.

WHAT YOU WILL DO
- Manage a caseload of clients
- Oversee a number of field caregivers
- Review and process incoming paperwork
- Enter data effectively and efficiently
- Respond to phone calls and emails
- Document client contacts
- Practice problem-solving techniques
- Conduct new client enrollment trainings
- Maintain a high level of customer service and compliance
- Speak in front of small groups of people
- Other duties as assigned
QUALIFICATIONS
- RN or LPN license required
- Strong written and oral communication skills
- Great time management skills to help successfully meet deadlines
- Proficient in working on a computer – specifically MS Office Suite, Adobe Acrobat, and Skype for Business
- Self-motivated worker with effective multi-tasking skills
- Valid driver’s license and reliable transportation
- Must be able to successfully pass a background screening
- Bachelor’s degree in the field of Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field preferred
- 1 year previous experience working with individuals with disabilities or the elderly preferred
- Combination of education and experience
