Department: Wellness & Nutrition

Reports To: Wellness and Nutrition Manager

Summary/Objective: The Senior Wellness Coordinator works with Wellness and Nutrition Manager to plan and facilitate individual and group programming for Omaha seniors. Under the direct supervision of the Wellness and Nutrition Manager the Senior Wellness Coordinator is responsible processing clients for rental/mortgage and utility assistance and overall case management. Additionally, the role will provide community outreach and collaboration. Confidentiality in this role is critical, since handling sensitive applicant information.

Essential Functions

  • Work with senior advisors to plan monthly events/activities. Engaging in outreach to seniors in Douglas County and surrounding areas and inviting them to ENCAP events, online activities, and office visits. Assist in promoting community interest in opportunities and benefits to senior citizens and community support of the programs/activities.
  • Take applications for hardship and food assistance. Assist in implementation of hardship assistance.
  • Interviews client and/or family to determine client’s needs for a variety of social services which may include employment, job search, physical and emotional needs, and basic skills remediation.
  • Provide pantries to seniors during distribution hours which may include the initial delivery to introduce yourself to the client. (Tuesdays and Thursdays 9AM-12PM)
  • Outreach by phone, zoom meeting, or in person to senior population to follow up and check in. Establishing effective working relationships with clients, family, community and professional sources.
  • Provide intensive case management, including individualized goal-setting, coaching, problem-solving, and follow up to all seniors participating in the virtual wellness cohort.
  • Entering data on computerized systems and maintaining case files in a timely manner and within prescribed deadlines.
  • Collecting and reviewing client information to determine application eligibility and/or options. Providing a variety of assistance to senior population; provide information of services available; serve as an advocate/liaison in assisting seniors in obtaining necessary social, medical, physical, financial, psychological, educational and nutritional services.
  • Provide timely and accurate benefits that improve nutrition and health as well as supplement basic financial needs.

Work Environment

This position is performed inside the office building and in outside weather conditions.

Physical Demands

This position is very active and requires standing, walking, bending, kneeling, stooping, crouching, and sitting at a desk. The employee must frequently lift and/or move items over 20 pounds.

Position Type and Expected Hours of Work

This is a full-time position. Occasional evening and weekend work may be required.

Travel Required

25% or less - You may be expected to travel for this position within the Douglas/Sarpy county area.

Preferred Education and Experience

  • 3 - 4 years of experience working directly with the aging population.
  • Bachelor’s Degree in Social Work, Human Services, or related field or 6+ years of experience in lieu of education may be considered.
  • Bilingual (English/Spanish)
  • At least 1 year experience with Microsoft Office applications (Outlook, Word, Excel, Access, etc)
  • 1 year experience working independently and meeting deadlines (self-motivated, good judgement, critical thinking, detail oriented)

Job Type: Full-time

Pay: $18.00 - $21.00 per hour
