This is a Full-Time position that will report directly to our Home Care Nurse Manager.
Saint Joseph PACE (Programs of All-inclusive Care for the Elderly) program provides care and services to seniors with chronic care needs allowing them to maintain their independence in their homes for as long as possible. The program offers a full range of health care services for individuals who are eligible for nursing home care but want to live at home. Saint Joseph PACE believes it is better for seniors with chronic care needs to be served in their home whenever possible. Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.
General responsibilities will include:
- Demonstrates nursing care to and instructs enrollees and families in treatments and other self-care interventions
- Participates in the development and revision of the plan of care working in collaboration with the interdisciplinary team; maintains timely and accurate documentation of services.
- Assesses health needs and contribute to the plan of care, initial implementation of nursing care plans and providing nursing care as well as periodic re-evaluation of individual and family nursing needs.
- Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants’ baseline status to appropriate staff daily.
- Assure that documentation for the clinic is accurate, current, and that the clinic documentation is up to date.
- Position does include an on-call rotation
Candidates will have:
- Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Registered Nurse in the State of Indiana.
- Minimum of two (2) years of demonstrated successful experience in related services with geriatric population.
- Active CPR certification or ability to obtain within 90 days of service.
- Valid driver's license in good standing and proof of insurance required.
- Must have the ability to perform standard clinical procedures according to established protocols and possess initiative, solid problem-solving skills and a high level of accuracy and thoroughness in documenting the delivery of services.
- Experience in the provision of nursing care, including assessment skills, health education, and current nursing treatments and protocols solid problem-solving skills and a high level of accuracy and thoroughness in documenting the delivery of services.
- Must possess strong interpersonal skills in order to interact in extremely delicate, sensitive and/or complex situations.
- Candidates should have excellent communication skills, with the ability to communicate with multiple levels in the organization.
- Must possess strong analytical and organizational skills. Must have the ability to meet strict deadlines with a high level of accuracy, ability to prioritize tasks in a highly automated setting and possess strong interpersonal skills.
- Candidates should also possess the ability to work as part of a team.
- Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system. Knowledge of systems necessary, experience preferred. Ability to type with speed and accuracy. Ability to use other software as required to perform the essential functions of the job.
