JOB SUMMARY

The Women Center RN will assist with coordination to ensure that patients, their family members and their caregivers navigate the continuum of care for breast care services at St. Mary Mercy Hospital. Assists and/or coordinates patient appointments for consults, diagnostics and other support services to facilitate timely coordinated care. Develops, implements and evaluates education materials and programs for patients and the community at large. Works under disease site medical director the Breast RN Navigator coordinates multidisciplinary planning conferences to review complex patient cases with a team approach in accordance with recognized treatment guidelines. Triages and assesses the medical, emotional and social needs of patients to respond to the unique needs of each patient and their family. Works with staff to collect data, monitor and track patient outcomes to develop and/or enhance quality of care.

PERFORMANCE DUTIES

  • Act as a link or “navigator” between patients and all other care providers and coordinates access to departments and physicians, including social workers, dietitians, diagnostics and other appropriate services.
  • Follows up in identified psychological and support needs of patients by making appropriate referrals to in-house and community services, problem solves and maintains working relationship with other members of the treatment team.
  • Explain the referral process with patient and facilitates and/or schedules appointments for further diagnostics, medical or surgical appointments or ancillary services if needed.
  • Responsible for outreach efforts to establish and maintain positive working relationship with key customers (e.g. primary care physicians, surgeons, radiologists, St. Mary Mercy Hospital staff).
  • Identifies physician preferences for communications between patient, primary care physician and specialists. Also identifies physician preferences for specialty referrals.
  • Works with the allied health staff in the Cancer Center, Women Center, Community Outreach Staff, Radiology, Physicians, to organize and deliver programs and services to support to patients and their families.
  • Under the direction of disease site Medical Director, coordinates multidisciplinary care conferences to review specified patient cases using a team approach. Conducts necessary preparation and follow-up.
  • Collaborates with physicians in the development and application of practice guidelines, quality improvement and research to enhance the quality of care to patients and their families.
  • Provides and/or facilitates patient care through the application of nursing process and implementation of physician orders within the framework of established policies, procedures, standards and guidelines for patients in specific clinical settings.
  • Involves and informs family members in the care of patients, coordinates family meetings with health care team members when appropriate.
  • Coordinates inpatient discharge planning with the oncology case manager and the patient’ s family, social workers, home nursing staff. Works with patients and family members to ensure discharge instructions are understood and followed.
  • Researches, evaluates, interprets and makes recommendations of findings for quality improvement and practice purposes.
  • Obtains and maintains knowledge and skills in area of specialty by reading journals, attending seminars, CME programs and professional conferences. Participates on hospital committees and teams to positively impact care of patients.

JOB QUALIFICATIONS

A.Education

  • Minimum BSN required; Master’s preferred

B.Job Knowledge

  • Oncology Certified Nurse preferred or attain within one year

C. Training and Experience

  • At least five years experience in nursing at least two in oncology (inpatient and outpatient setting).
  • Successful completion of a Nurse Navigation Course.
  • Experience in breast cancer/Women’s Health preferred.

D. Job Knowledge & Skills

  • Strong listening and communication skills; strong documentation skills; requires excellent organization, written and verbal communication skills. Masters the ability to manage a variety of tasks and problems simultaneously; high level of initiative and ability to self-direct to solve problems and troubleshoot; ability to work collaboratively with multidisciplinary group of providers. Possess excellent interpersonal skills.

DEMANDS

A.Physical

  • Must be able to work with a high level of activity, interruptions and prioritize tasks daily. Ability to walk around to locations throughout hospital and be available for some weekend and/or evening services or programs. Requires some flexibility in hours to meet deadlines or patient’s schedules.

B. Special
  • Extensive use of keyboard, phone, some lifting required.
