Essential Job Duties:

• Meet clients and families in their home setting
• Travel by car to multiple boroughs, focus Manhattan, Brooklyn, Queens
• Develop a plan of care including nursing and support services that meet the client's needs
• Determine what care giver team members are appropriate to meet the client's needs and manage that team ongoing
• Available to client and team on a daily basis to ensure services are provided timely and in full compliance with the clients stated desires.
• Always strive to ensure 100% client satisfaction.

Educational/Experience Requirements:

• Registered Nurse. BSN preferred
• Prior home care experience REQUIRED
• 3+ years clinical experience
• RN License
• Valid US Drivers License

Job Type: Full-time

Pay: $90,000.00 - $120,000.00 per year
