Position Summary
The RN Navigator serves as the primary point of contact and is responsible for coordinating the care and services of the identified patient populations across the continuum of care. The incumbent promotes effective resource utilization and assumes a leadership role within a multidisciplinary team to identify and eliminate barriers to care in order to achieve an optimal clinical outcome for the patients and caregivers.

Minimum Education Requirements
Bachelor of Science in Nursing (BSN)
High School diploma or GED.

Minimum Experience Requirements
3 years of experience, with one year in specialty.

License/Certifications
Current state RN license.
Current Basic Life Support certification from American Heart Association.
Certification required within specialty, preferred.

Physical Requirements

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly
