The Health Promotion and Disease Prevention (HPDP) Program Manager position is in ACC Nursing Service at the VA Palo Alto Health Care System (VAPAHCS), Palo Alto, California. The HPDP Program Manager is a professional nurse who will serve as a facility content expert for the HPDP program and oversee the integration of HPDP services into patient care throughout the facility and affiliated community-based outpatient clinics (CBOCs).The incumbent will be supervised by the Whole Health Program Director, under ACC Nursing, who provides the administrative and clinical supervision, general instruction, plans and assigns work, assigns time schedules, approves leave, and completes performance evaluations. Incumbent is responsible for being self-directed in the delivery of patient care under minimal supervision and instruction by a registered nurse.
Work Schedule: Monday-Friday: 8:00am to 4:30pm with flexibility to meet patient and program needs.

The major duties and responsibilities include, but are not limited to:
- The incumbent will serve as a facility content expert for the HPDP program and oversee the integration of HPDP services into patient care throughout the facility and affiliated community-based outpatient clinics (CBOCs). The HPDP Program Manager must be knowledgeable about HPDP as well as outreach and local community resources.
- The HPDP Program Manager is required to perform statistical reporting, evaluation, consultation, and collaboration with non-Department of Veterans Affairs (VA) facilities as well as other VA health care professionals and administrators. The incumbent leads efforts to identify HPDP challenges, priorities for improvement, and opportunities for disseminating and sustaining best practices. The incumbent serves as primary facility point of contact for local VISN and national health promotion campaigns and events, ensuring local implementation and participation. The incumbent uses professional standards of practice and care based on the best available scientific evidence to evaluate HPDP programs, services and activities.
- The incumbent advises facility and primary care leadership on clinical and administrative issues related to HPDP, including clinical preventive services. The incumbent advocates for policy and program development and resource allocation to support HPDP programs and services for Veterans. The incumbent provides leadership in the development or modification of service delivery processes to support the successful implementation of HPDP programs and services. The incumbent designs, develops, and facilitates HPDP outreach events including strategies to evaluate outcomes based on the best available evidence. The incumbent develops or coordinates the development of user-friendly tools to assist in successful implementation of HPDP programs and services. The incumbent ensures that educational opportunities are provided to clinical and administrative staff members to support and enhance their involvement in and delivery of local HPDP programs, services and seminars and serves as an educational resource for issues related to evidence based HPDP.
- The incumbent routinely assesses the prevention-related educational needs of both patients and employees, develops educational programs and learning outcomes, coordinates the development, delivery and evaluation of those programs and recommends and implements changes at the program or service level to meet HPDP standards. The incumbent maintains and chairs a facility interdisciplinary HPDP committee and serves as a facility content expert in HPDP, including clinical preventive services. The incumbent collaborates with primary care and all clinical and administrative services as needed to ensure that HPDP programs and services are available and maintains effective working relationships with other facility staff responsible for related programs and services, including the Health Behavior Coordinator (HBC), Move!, Veterans Health Education, My HealtheVet, Women Veterans Program, Quality Management, Public Affairs, Social Work, Voluntary Service and others. The incumbent serves as the facility liaison to the VISN, VHA National Center for Health Promotion and Disease Prevention (NCP), and VHA Central Office program office in matters related to HPDP. The incumbent develops relationships with external stakeholders and agencies, including Veterans Service Organizations and community agencies, to enhance and support HPDP programs and services.

The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
QualificationsTo qualify for this position, applicants must meet all requirements by the closing date of this announcement, April 27, 2021.
Basic Requirements:- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:- Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
- Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
