Position Focus:

In 1971, Yale University began a unique and historic experiment providing health services to its facility, staff and students through a multidisciplinary health maintenance organization located on campus. Now, 47 years later, Yale Health has more than 37,000 members including students, staff, faculty and their families who come from every state in the country and almost every country in the world.

Our state-of-the-art facility at 55 Lock Street, built in 2010, is where our members receive most of their care. It is a 144,000 square foot medical facility with over 90 exam rooms, a 24/7 Acute Care Department, a 17-bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MR and CT scan, x-ray and ultrasound and a full-service retail pharmacy. It houses our 150+ providers, including 111 primary and consulting physicians.

The Certified Diabetes Care and Education Specialist Advanced Practice Provider position delivers direct patient care, expert care management, chronic disease management, patient education, and clinical expertise. This person will help us to improve the quality of care Yale Health is able to provide for our patients with pre-diabetes, diabetes, and gestational diabetes. This position maintains 4 clinical sessions per week and is directly involved with the care of over 450 patients across the organization, in addition to being a resource for the care of many others.

Participate in direct care and outreach, working on a quarterly basis with each clinician on each patient with a hemoglobin AIC of 8 or above, a marker of poor diabetes control.

Guide the care of patients using insulin pumps combined with continuous glucose sensors, as well as patients using glucose monitoring sensors for safe insulin use.

Provide education and management of initiation and titration of insulin and GLP-I agonist therapy.

Contribute to the positive morale in the workplace and model professional behavior at all times.

Participate and contribute to an active learning environment.

Often alternate visits with the PCP or the endocrinologist to extend care, as well as provide additional support to our patients at YHC.

Maintain a computer with pertinent programs providing ability to upload insulin pumps, sensors and meters; share/review reports with patients or send to endocrinologists for review.

Serve as the main contact person with DME suppliers, in direct communication with Byram, Medtronic, Edgepark, or Tandem, coordinating orders, working with clinical support personnel for initiation of insulin pump therapy. Help trouble shoot problems with the patients for their various supplies as needed.

Manage patients monthly for the department of Obstetrics/Gynecology, who have gestational diabetes, meeting with these patients either weekly or biweekly, assisting with referral to Maternal Fetal Medicine as appropriate.

Serve as a local resource for Student Health in guiding assistance with new diagnosis of DM, pre-diabetes, and oral antihyperglycemic agent/insulin therapy guidance.

Participate in a Diabetes Task Force, serving as a liaison between Yale Health and the Endocrinology Division, keeping Yale Health up-to-date on latest treatment guidance as well as connecting to other resources and services at Yale and the broader community.

Lead patient education efforts, including the Diabetes Prevention class and the Diabetes Education Series on a monthly to quarterly basis.

Essential Duties

1. Performs physical assessments of patients, including triage duties and advises on treatment or course of action. 2. Identifies and evaluates nursing services problems and develops and implements appropriate programs and services, including assessing staffing needs and arranging coverage as needed. 3. Develops, implements, and manages nursing care plans in collaboration with physicians. 4. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 5. Provides leadership, guidance, direction to clinical staff, and collaborates with nursing at all levels. 6. Determines standards for quality assurance and infection control procedures; trains staff in universal and isolation precautions in the area of infection control. 7. Performs diagnostic and therapeutic measures in collaboration with physicians. 8. Evaluates the quality and effectiveness of nursing practice and nursing services. 9. Records patients’ medical histories and symptoms, performs diagnostic tests and analyzes results, operates medical machinery, administers treatment and medications, and coordinates with patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines. 10. May perform other duties as assigned.

Required Education and Experience

Bachelor’s Degree in Nursing and two years’ experience or an equivalent combination of education and experience.
