RN Case Manager - Registered Nurse

Overview: The RN Case Manager -Registered Nurse (RN) provides for and assists in meeting the senior residents’ physical, emotional and psycho-social needs while adhering to facility, state and federal guidelines.
This nursing schedule includes;

• 64 hours in two weeks
• Day/Evening Shift Hours
• Every other Sat-Sunday and rotating holidays

The Pillars of Mankato, Minnesota provides independent living, assisted living and memory care apartments for seniors in a specially designed community. The Pillars incorporates the seven components of wellness (emotional, physical, intellectual, spiritual, social, environmental and occupational) into our 118 units that make up The Pillars. Responsibilities/Job Description:

• Maintains current assessment and documentation using progress notes of resident health status
• Implements new and updates current resident care plan
• Coordinates resident health services with internal/external providers and getting orders as needed
• Manages and oversees timely and accurate medication and treatment administration
• Acts as support to Director of Health Services to fill in as RN on call during scheduled times
• Monitors work and performance of licensed and unlicensed staff

Qualifications: Required:

• Current MN RN license or by start date
• AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers

Preferred:

• Work experience in a health care setting
• Good written and verbal communication skills to interact with residents, families and physicians or providers
• Ability to work with and have sensitivity to the needs of the geriatric population
• Experience with mental health clients very helpful
• De-escalation, therapeutic communication, conflict management helpful
