Full-time with Minimum On-Call Rotation

Coverage Areas: Porter, La Porte, Starke, Jasper and Lake Counties, mileage paid at the IRS rate.

$4000 bonus.

Job Duties
  • RN Case Manager will provide direct patient care to hospice patient case load.
  • Demonstrates comprehensive knowledge and skill in current nursing practice.
  • Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
  • Functions independently and appropriately, and demonstrates the ability to deal effectively with the demands of the job.
Position Summary

The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients in their home. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.
