What We Do
San Francisco is a pioneer in homeless services and a leader in providing supportive housing as a permanent exit from homelessness. The Department of Homelessness and Supportive Housing seeks to be a national leader in the movement to end homelessness by developing a coordinated, client-focused system of services, piloting innovative models, and implementing proven solutions with measurable results. Major programs include: street outreach and service connection through the Homeless Outreach Team; 1,500-bed shelter system for adults and families including shelters for members of the LGBT community and survivors of domestic violence; Navigation Centers that provide temporary shelter to chronically homeless individuals using a low-threshold entry model; rapid rehousing rental subsidies for families, adults, seniors and transitional aged youth; the Homeward Bound program which has helped 10,000 individuals return to stable housing in their city of origin; and robust supportive housing programs of nearly 8,000 units which provide permanent housing and services to formerly homeless individuals and families.

Job Description


APPOINTMENT TYPE:

This is a Temporary Exempt (TEX) appointment. This position is excluded by the Charter from the competitive Civil Service examination process and incumbents are considered “at will” and shall serve at the discretion of the Appointing Officer. This position has an anticipated end date of October 2022 (use only for certain COVID related positions)

What You’ll Be Doing


Working within the Rehousing Unit of HSH, the Resource Coordinator supports and assists the guests of the Shelter-In-Place (SIP) Hotels to prepare and follow through with the housing options that are available to them in advance of SIP Hotel transitions or closures. The role involves coordination with the site provider’s Care Coordinators, engagement with guests and collaboration with the overall Rehousing program’s systems.


Example of Essential Duties and Responsibilities:


  • Treat SIP Hotel guests and site providers with dignity and respect.
  • Provide ongoing support to the Care Coordinators with respect to the Rehousing Process.
  • Serve as a resource on Rehousing questions.
  • Be a point person to handle guest Rehousing questions.
  • Seek information via appropriate channels to answer questions.
  • Triage ONE System data quality concerns.
  • Identify and work to engage with guests who have complex needs.
  • Coordinate with Care Coordinators looking for assistance to connect with specific guests.
  • Request clinical and/or behavioral health support via the appropriate channels as needed.
  • Prepare Rehousing-eligible guests for the Rehousing Process.
  • Assist in the communication of placement options with specific guests.
  • Facilitate coordinated entry assessments for those guests who still need one
  • Have guests in the Pandemic Prioritization Queue (Categories B - D) AND/OR guests eligible for Rapid Rehousing, Flex Pool Scattered Site housing, and Short Term Housing Assistance sign a Rehousing Participation Agreement.
  • Upload signed Rehousing Participation Agreements in the ONE System.
  • Work with Care Coordinators and Coordinated Entry staff to get Rehousing-eligible guests documents ready
  • Check in with guests as to their progress on gathering documents.
  • Answer guest questions about documents
  • Upload any documents from iFiles, CalWIN, and/or MEDS as available into the ONE System.
  • Scan and upload any documents acquired into the ONE System.
  • Have guests sign a Rehousing Participation Agreement after explaining the Rehousing Participation Policy to them, inclusive of the use of translation services and/or auxiliary aids as appropriate.
  • Upload signed Rehousing Participation Agreements in the ONE System.
  • Track refusals to sign ROIs, Agreements or participate in document readiness in the ONE System.
  • Via the Client Engagement team manager, identify and prioritize the work based on building closing dates, the Pandemic Prioritization Queue and other factors that affect the work.
  • Maintain good relationships with the site providers, including the Site managers and Care Coordinators. Report any concerns to the Client Engagement manager or your team lead.
  • May include additional duties as assigned.


Qualifications


MINIMUM QUALIFICATIONS

Applicants must meet the following minimum qualification requirements by the final filing date unless otherwise noted.

Education:
Sixty (60) semester units or ninety (90) quarter units from an accredited college or university OR

Experience:
1. Two (2) years of clerical work experience which includes the following: use of computer applications to input and retrieve information; regular public contact to provide assistance or acquire detailed personal or confidential information; and interpreting and applying rules, regulations and policies; OR

2. One (1) year of experience determining eligibility for health and/or social services programs, loans, financial assistance, unemployment or veterans benefits.

Substitution:
Thirty (30) semester units or forty-five (45) quarter units from an accredited college or university may substitute for one (1) year of the work experience under experience requirement number #1.
