Intercept Health is now hiring a Full Time Program Manager for Fresh Start. This individual oversees all staff and direct care issues and services, ensuring effective leadership-based management of the program with an emphasis on high-quality care delivered in a cost-effective manner by performing the duties outlined below.

Education
& Experience

Must be registered as a QMHP-C or Trainee. Bachelor's degree in a human services field with five or more years of experience with children, two of which are in a residential facility; OR Master's degree in a human services field with three or more years experience with children, one of which is in a residential facility; OR Two or more years in group home management/supervision or similar experience.

Valid Driver's License and favorable driving record (no more than 3 moving violations in the last 3 years; no DUIs/Reckless driving in the last 3 years)


Essential duties include the following. Other duties may be assigned.

  • Helping the resident and parent and/or legal guardian understand the effects of separating a resident from the family, as well as what he/she can expect in terms of effect of group home living. Assist in establishing and/or maintaining a relationship with his/her family and to effectively plan and prepare for the resident's future care.
  • Identify and maintain contact with community resources
  • Utilizes knowledge of CSA, social services, and treatment modalities in presentations to FAPT team and public relations opportunities
  • Attends all FAPT review meetings and reports on resident's therapeutic progress and discharge plans
  • Regular discussions of residents' issues, progress with social workers and probation officers
  • Meet with social workers and probation officers for Quarterly Progress and discharge planning
  • Contacting and facilitating communication with parents (i.e., TOLAs)
  • Review medication needs of each resident, ordering or deleting medication as prescribed by the physician
  • Accepts full and direct supervisory responsibility over Staff Supervisor, Day Counselor and Living Skills Counselor as well as oversight responsibilities for all staff members.
  • Coordinates/facilitates house staff meetings

  • Works in cooperation with Human Resource Manager to provide and/or arrange for in-service and other training for staff on relevant topics and new employee hiring/interviewing.

  • Familiarizes self with federal, state, local, and regulatory requirements including Interdepartmental licensure standards and ensures Intercept House's compliance.

  • Accepts full responsibilities for effective execution of behavior program within home.

  • Meet with residents to address any health-related issue, setting any type of doctor, dentist appointments as needed
  • Coordinate placement with the central school office, i.e., pupil placement office, special education, degree of restriction, GED for residents' educational purposes.
  • Meet regularly and/or receive educational progress reports from resident's teachers. Assist in the intervention of school crisis situations and attend all parent/teacher conferences
  • Focus on instruction, guidance, and competency training of each level's independent living skills and strategies, using both group and individual instruction
  • Meet weekly with each resident to encourage level packet progress
  • Monitor residents' work schedules and performance. Assist residents in locating employment (job search).
