Residential Medicaid Case Manager

Under the assigned supervisor, provides direct casework management, counseling, or mental health services to children and families within the agency’s policies and program context, utilizing human service principals in developing relationships, assessment, and development of service plans.
Required Qualifications
- Education: BA/BS degree in psychology, sociology, social work, or related field.
- Certification: Medicaid and MHP
- 50% Driving Weekly
Job Duties
- Arranges for and provides transportation for youth, such as school appointments, medical appointments, court and ACRs
- Refers and links youth to necessary resources to meet needs, including external therapy, medical and educational resources, and psychological and psychiatric specialists
- Arranges visits with youth with families, including siblings, and visiting resources
- Assists with the completion of Mental Health Assessments, Individual Treatment Plans, Individual Treatment Plan Reviews, and Clinical Summaries; complete Discharge Summaries
- Participates in individual treatment plan meetings, reviews, additional staffings, transition staffings, and child and family team meetings
- Participates in court proceedings on behalf of the youth; this includes providing testimony at hearings, compiling court reports, and preparing case records
- Completes and participates in Administrative Case Reviews
- Assists youth in determining transition and discharge plans
- Acts as a liaison with multiple public and private agencies to advocate and assist with the service needs of the youth
- Continually trains and maintains certification in the implementation of Therapeutic Crisis Intervention (TCI) or other approved behavior modification program
- Follows OHU Residential Program Plan, including daily schedule, administration of consequences, and mental health treatment of youth in residential care
- Demonstrates the highest investment in and commitment to quality improvement practices in order to continuously improve the delivery of services to our residents
- Participates in a weekly Clinical Team Meeting of assigned cottage; moderates staff team meeting of a residential home
- Participates and contributes to a treatment team of a residential home, including attending weekly team meetings and staffings, as required
- Attends training, seminars, and conferences as assigned by supervisory or administrative staff
- Completes in a timely and accurate fashion all documentation required for the position
- Treats with absolute confidence all information concerning agency residents and programs
- Transports clients (youth) and their families to mandated court hearings, school appointments, doctor appointments, or other case-related appointments
- Perform on-call duties as assigned
