Job Description

POSITION SUMMARY:

Provides direct operational and financial leadership and accountability for the daily execution of all activities that ensure the highest level of care and service for residents, families and team members. Functions as a resource person and role model for the care team. Proactively support all efforts to ensure delivery of safe care and services. Reports to the Executive Director

QUALIFICATIONS, SKILLS & KNOWLEDGE REQUIREMENTS:

1. Experience in care management typically gained by leadership experience in a supervisory or management role in a healthcare, personal care or assisted living environment.

2. Experience and training in caring for residents with dementia and related disorders.

3. Essential Skills:

a. Leadership – Ability to set priorities and motivate Care Coordinator, which requires self-awareness, self-management, social awareness and relationship management. Be a source of energy, empathy, and trust. Work daily to develop Care Coordinators through positive and constructive feedback and coaching.

b. Communication – Effective communication in all applications, including one-on-one, small and large groups, email and social media. Realize importance of active listening.

c. Collaboration – Serve as role model for working together. Support cross-functional efforts and model collaborative behaviors to set the example for your caregivers.

d. Critical Thinking – Understand where and how your projects fit into the bigger picture to enhance your effectiveness. Review priorities in light of larger goals, and translate into

meaningful goals for caregivers.

e. Finance – Know the language of numbers and their impact on financial performance.

Imperative to learn and apply the basics in monitoring and controlling expenses and budget.

f. Project Management – Understand project management practices to ensure timely

completion and proper control of initiatives.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Learning Agility

a. Looks for and is open to information challenging assumptions and conclusions on resident care.

b. Works with Care Coordinators to analyze issues and mistakes to improve systems.

c. Is willing to challenge “the way it has always been done”. Brings new ideas and best practices to the team for them to learn and apply.

d. Partners with Human Resources Manager to coordinate required and necessary training.

e. Assures the property is meeting its compliance requirements.

f. Pursues professional growth by participating in 15 hours of job related educational development per year

2. Business Acumen

a. Accountable for the development and management of area budgets within the established fiscal guidelines and facilitates the monthly analysis of budget performance for areas of responsibility.

b. Oversees all components essential to effective operations, ie. care coordination resident/family experience, quality/ safety, and people management

c. Partners with Human Resources Manager and supervises Care Coordinator to assure proper position control is maintained and necessary staff are onboard and available to work.

d. Supervise Care Coordinator in building schedule and mainting a staffing level to provide proper care to our resident population

e. Participates as needed in the development, planning, marketing, implementation and evaluation of programs and services

3. Comfort with Ambiguity

a. Evaluates new or unique situations to determine whether a situational or systemic approach is required

b. Pushes forward with important initiatives in the face of uncertainty by using sound judgement and filling in details as more is learned

c. Knows when to settle for an 80% solution and when to push for 100%

d. Acts as the community leader when the Executive Director is not available

e. Oversees monthly care meetings and assures pertinent information is presented.

f. Shows presence in the community to show support for all three shifts.

g. Leads teams with confidence in ambiguous situations, helping their team to push forward

4. Planning and Organization

a. Understands, plans for and secures appropriate budget and resources to ensure initiatives and teams are set up for success.

b. Establishes clear goals and objectives for the team and clarifies roles and responsibilities to ensure successful achievement.

c. Takes a business focus, end-to-end approach to analyzing issues and developing options.

5. Drives for Results

a. Directs the care components and outcomes of assisted living and memory care

b. Uses an understanding of the Resident/Family to develop plans and priorities to deliver a high level of service and resident experience

c. Oversees the on-boarding of residents from assessment, to care planning and all follow-up coordination of care and services

d. Ensures the team has the resources they need to exceed resident and family expectations.

e. Stays close to the most significant priorities of the community, monitoring progress, developing alternatives and contingencies as needed.

f. Directs development of care standardization, and monitoring of performance improvement, quality measures and quality controls for both AL and MC

g. Oversees and ensures compliance with all regulatory requirements, community standards, and policies and procedures

h. Identifies potential areas of risk and oversees team implementation of necessary actions

i. Removes barriers and identifies patterns for improvement Responsible for resident assessments and corresponding care plan implementation.

j. Maintains open and timely communication with residents and family members to assure resident needs are being met.

k. Resolves issues or obstacles getting in the way of staff accomplishing goals and objectives

l. Monitors and ensures compliance with all regulatory requirements, community standards and policies and procedures

m. Implements initiatives to deliver care standardization, and performance improvement

6. Optimizes Resources

a. Sets goals and standards for the team that require the staff to “up their game” every year.

b. Considers cost, efficiency and value when making changes to staffing or processes which affect team members or staff.

c. Is thoughtful and realistic setting budgets and resources, always looks to reallocate budget and resources from lower valued activities to higher priorities.

d. Fosters collegial relationships between team members and all levels of the community to assure high quality, and cost effective care and services.

7. Builds Community Capabilities

a. Responsible for all aspects of managing and leading the team including: interviewing, hiring, training, developing, directing work and processes, managing performance, recognizing and reqarding employees

b. Focuses time and attention on key retention activities

c. Mentors leaders and team members to encourage personal and professional growth though constructive performance appraisals and development plans

d. Proactively provides developmental experiences and takes action when staff are becoming stagnant in their roles.

e. Ensures best talent in best roles by accelerating development through new challenges and experiences.

f. Ensures the team has the right mix of capabilities needed to accomplish objectives. Takes action to build capabilities to fill gaps.
