UW Neighborhood Clinics (UWNC) is a network of community-based clinics throughout the Puget Sound region. The clinics offer a wide spectrum of primary care services, as well as secondary care, from pediatrics to geriatrics. Urgent care services are offered seven days a week at select locations. UWNC - AMBULATORY NURSING has an outstanding opportunity for a per diem (0%, variable hours per week), day shift, TELEPHONIC SERVICES NURSE (job profile: Registered Nurse 2) to join our Northgate location. The Telephonic Service Nurse will be responsible for triaging incoming calls for patients, assessing needs, giving appropriate clinical options, and facilitating requests to primary care providers, specialists, health care facilities and community resources. The Telephonic Service Nurse will also be responsible for the coordination of complex medical care, particularly for the anticoagulation service patients.

SCHEDULE:
The Telephonic Services Nurse will work at our Northgate Office, variable hours per week, day shift only. This position has the potential to work from home after working successfully and independently for 4-6 months.

RESPONSIBILITIES
Telephone Triage
  • Performs telephone triage and provides health information using approved protocols and materials.
  • Documents nursing process reflective of data collection, assessment, triage disposition, and plan of care.
  • Conducts thorough and sound nursing assessments of caller’s presenting symptoms.
  • Develops implements and evaluates a plan of care for each caller’s presenting symptoms.
  • Applies nursing process to identify and prioritize healthcare triage interventions that are appropriate and accurate, effective and reduces the risk of adverse outcomes.
  • Documents all call inquiries according to policy and procedure.
  • Provides transitional care services to patients with moderate or high-complexity medical decision making during transitions in care from an inpatient hospital to an outpatient setting (skilled nursing facility, assisted living, home, etc).

    Anticoagulation Services
  • Provides patient care and telephonic management for anticoagulation therapy patients at UWNC.
  • Evaluates INR responses, identifies risk factors, and facilitates brief interruptions in therapy across various disciplines and venues
  • Collaborates with UWNC Providers to ensure appropriate and safe anticoagulation therapy management.
  • Coordinates care with internal and external resources, i.e. physicians, home care agencies, nursing homes.
  • Assists in the development of systems and processes to promote quality and manage operational costs.

    REQUIREMENTS:
  • Anticoagulation Management Certification required within 6 months of hire.
  • 3 years of nursing experience

    LEGAL REQUIREMENTS:
  • Current license to practice as a registered nurse in the State of Washington

    DESIRED QUALIFICATIONS:
  • Previous nursing experience, preferably in care management, telephonic triage, or anticoagulation management
  • Bachelor’s degree in nursing (BSN)
  • Ambulatory Nurse certification
