Our New Jersey Facilities
Nursing & Rehab in Brick provides both short-term and sub-acute rehabilitation programs for patients as an alternative to prolonged hospital care. In this 137 bed facility patients receive various services on site in order to eliminate re-hospitalization. Brick is the only skilled nursing facility in New Jersey to receive the Advanced Certification in Heart Failure from the Joint Commission in conjunction with American Heart Association. It’s conveniently located near Parkway Exit 91, just down the street from Ocean Medical Center.
Manor By the Sea in Ocean Grove is a 147 bed inpatient nursing & subacute rehabilitation facility providing personalized medical care programs for Long-Term and Short-Term Nursing & Rehabilitation to patients of all ages. Our services are an alternative to prolonged hospital care. The facility provides easily accessible living space with luxurious amenities and beautifully decorated private and common areas which make our patients feel at home. The subacute unit offers the latest technologies, design, equipment and spaciously well-appointed rooms, including private bedrooms and living space for bariatric and palliative care patients. This location is easily accessible by public transportation, located right between Bradley Beach and Asbury Park.
Nursing & Rehab in Wall is strictly a rehabilitation facility with 126 beds caring for patients of all ages whose length of stay can range from three days to several weeks. This facility has achieved the Pathway to Excellence designation by the American Nurses Credentialing Center, signifying based on team member job satisfaction that nurses can flourish and feel empowered in their careers. Also, this facility is CARF Certified (Commission on Accreditation of Rehabilitation Facilities), which means that standards for providing top-quality services and the best rehab care have been achieved. HMH Nursing & Rehab at Red Bank is a newer addition to the HMH Family, located on a quiet street within minutes of Garden State Parkway Exit 109. In this 180-bed sub-acute and long-term care facility, skilled professionals provide care for residents age 35 and over who have complex plans of care. This facility works closely with the Riverview Medical Center and is Medicare and Medicaid-approved.

The Hackensack Meridian Nursing and Rehabilitation (HMNR) RN Nurse Residency Program is a twelve-month program developed to expand upon critical thinking capacity of new RNs as they are guided and supported in the transition from student nurse to RN practice. Nurse Residents (NR) are expected to be active participants, fully engaged in the learning process and committed to completing the requirements of this twelve-month program. There are two tracks: one for new to practice licensed RNs and one for seasoned Rehab LPNs who have attained an RN License. At the completion of the twelve-month program, there is a graduation ceremony and a monetary award of $1,000 for each graduate! After graduation, the RN will be eligible to apply for the Senior Nurse position, which is an introduction to Leadership responsibilities/roles.
Hackensack Meridian Health Nursing and Rehabilitation's (HMNR) clinical ladder program, signified as the Clinical Advancement and Recognition of Excellence or C.A.R.E., is an outcome based clinical recognition program for which clinical nurses are acknowledged and rewarded for achieving and demonstrating increased levels of expertise, and for their ability to make an impact on resident/patient outcomes at the bedside. All part-time (over 20 hours/week) and full-time clinical nurses who have met expectations on the Annual Performance Appraisal are eligible to apply after one (1) year of service. This includes all "hands-on" RNs and LPNs, including unit managers, supervisors, and case managers. Additional compensation of $1 per hour is provided to approved participants who satisfy the activity requirements and eligibility standards of this program.
In addition to these programs, RNs receive additional compensation of $2 per hour for obtaining their BSN, and all nursing roles are eligible for additional compensation for earning and maintaining up to two approved nursing certifications at $1 per hour for each certification.

A day in the life of a Registered Nurse or Licensed Practical Nurse at Hackensack Meridian Health can be described as follows:
- Functions responsibly as an educator; teaches patients and families based on identified health needs.
- Utilizes appropriate teaching methodologies based upon the learning needs of the patients.
- Performs as a practitioner in a safe and efficient manner.
- Actively perpetuates assessment, planning, intervention and evaluation of resident care plan are reflective of Policy and Procedure and Standard of Care, measured by complete and accurate documentation.
- Demonstrates team approach to problem solving through frequent communication and collaboration on complex issues.
- Monitors and supervises nursing staff to ensure quality of care.
- Maintains all facility equipment as per facility policy. Follows Manufacturers’ specifications for use of equipment.
- IV pumps
- Identifies equipment problems and safety hazards and takes appropriate action to initiate repairs and/or correction of deficiencies.
- Supplies are stocked and cleaned as appropriately so equipment is readily available.
- Delegates tasks and assignments to appropriate staff within their scope of practice.
Qualifications:
- Current and valid NJ RN or LPN License is required
- Experience with geriatric patients is preferred
- CPR/BLS Certification by American Heart Association is required
