Job Description:

  • Develop and maintain contact with key hospital and other facility discharge planning services and/or management to provide ongoing updates on Company's Hospice services available in a market. Promote exemplary customer service to physicians, facility personnel, client and client family at all times.
  • Primarily conduct pre-discharge hospital visits at the physicians' request to determine the eligibility for hospice.
  • Ensure effective communication and collaboration with branch staff and field sales resources via weekly meetings. Actively participate in weekly branch sales / operation meeting, bringing relative data, reports to meeting for review as well as information regarding changes within accounts and referral sources.
  • Communicate with RN and Account Executive when patients are admitted in their territory.
  • Assist branch in timely processing of physician orders.
  • Inform hospital personnel, patient and / or family of acceptance to Hospice and documents all demographic, clinical and payer information, on company approved forms to support diagnosis and hospice orders. Provide input to care plan development and informs the Company location clinical staff of patient's acceptance and ensures that the patient's needs are appropriately matched.
  • Compliance to technology processes and expectations.
  • Copy all medical records pertinent to Hospice care, arrange DME equipment and secure necessary prescriptions for medications at home.
  • Follow all current patients in the hospital setting through their hospital stay. If a patient is admitted for a non-related diagnosis, evaluate the course of care to confirm that the care remains non-related to the terminal diagnosis.
  • Assist with discharge planning by communicating with the Hospitalist/attending physician regarding the discharge, patient care and medications.
  • Follow up on all referrals not admitted to the hospice program.

Qualifications:

  • Bachelor's Degree or ASN in Nursing.
  • Registered Nurse Licensure required in state of practice plus a minimum of three years clinical experience.
  • C.H.P.N certification is highly desirable.
  • Strong knowledge of governmental hospice regulations, Medicare requirements and care plan development techniques required.
  • Understanding of hospice philosophy and the principles of death and dying.
  • Excellent customer service, organization, time management, problem solving and communication skills required.
  • Demonstrates the ability to work independently.
  • Microsoft Office proficiency required.
  • Ability to appropriately represent Company service capabilities to the assigned referral source audience.
