- Provides leadership in the application of the nursing process to comprehensive Resident Assessment Instrument (RAI)/MDS assessment of residents in the CLC.
- The RN possesses the knowledge and skills necessary to effectively apply all aspects of the nursing process within the collaborative, interdisciplinary practice setting of the Community Living Center. This broad based clinical knowledge enables the RN to effectively implement health promotion and prevention interventions, manage acute and chronic illness states, assist veterans in the attainment of optimal levels of functioning through rehabilitation, and provide palliative care for the resident and their significant others.
- Functions as the coordinator and facilitator of RAI/MDS assessment processes according to Joint Commission Standards of Long Term Care; contributes to VISN Performance Measures.
- Maintains professional knowledge knowledge and skills based on current evidence-based practice.
- Self-directed in evaluating progress toward professional career goals (includes completing self-evaluation including personal goals).
- Uses professional standards of care and practice to evaluate the MDS assessment process.
- Demonstrates professional (i.e., respect, civility, ability to accept constructive criticism), effective communication and interpersonal skills.
- Promotes resident, family and team interactions and problem solving by actively participating in interdisciplinary treatment team meetings to facilitate collaboration and the achievement of identified goals.
- Facilitates an integrated resident centered plan of care.
- Coordinates with and assists others within or across work groups to validate and/or improve practice by applying current research-based knowledge or conducting research that may involve RAI/MDS data or processes.
- Facilities investigation and development of best practices in RAI/MDS process utilizing benchmarking or other qualitative and quantitative techniques.

Basic Requirements:- United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- Have at least a Diploma or an Associate Degree/Diploma in Nursing; BSN or Bachelor's Degree in healthcare related field preferred.
- Have at least 2 years of increasingly complex RN nursing practice.
- Have/maintain current certification in BLS/CPR.
- Be acquainted with long term care clinical environments including familiarity with the Minimum Data Set and Interdisciplinary Treatment plans.
- Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
- Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
- Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
- Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
Physical Requirements: To perform in a fully successful manner, the individual must have the physical ability and/or attributes to perform the following job-related duties: moderate lifting (15-44 pounds); moderate carrying (15-44 pounds); straight pulling (0-6 hours/day); pushing (1 hour/day); reaching above shoulder; use of fingers; both hands required; walking (0-5 hours/day); standing (0-6 hours/day); repeated bending; prolonged sitting up to 4 hours; both legs required; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; both eyes required; depth perception; ability to distinguish basic colors; and hearing (aid permitted). In addition, the following environmental factors may impact the job: walking outside and inside; working with hands in water; working closely with others; protracted or irregular hours of work (may be required to rotate to alternate shifts including night tours). A health examination mist be successfully completed prior to this assignment and periodically thereafter as required by the Occupational Health Clinic policy. There will be potential for exposure to blood and body fluids. The position is located in Inpatient Geriatric Services Division, in the Directorate for Nursing Services, Office of Chief Nurse Executives at Captain James A. Lovell FHCC in North Chicago, IL. FHCC is a larger than just a single facility, but rather a system of facilities throughout Northeastern Illinois which provide medical and dental services to U.S. Military Active Duty, Recruits, Veterans, and eligible beneficiaries in the Nation's first Federal Health Care Center.
