Description/Job Summary
The mission of Resources for Human Development (RHD) is to provide caring, effective and innovative services that empower people of all abilities as they build better lives for themselves, their families and their communities. RHD's multi-faceted programs serve people with a variety of challenges that include intellectual and developmental disabilities, mental illness, substance abuse and homelessness.
The Community Transitional Recovery Program (CTRP) Nurse provides medical and psychiatric monitoring and direct care support via individual and group sessions. The services are designed for individuals diagnosed with a severe and persistent mental illness and having met the criteria for participation in the CTRP program.The responsibilities of the nurse include but are not limited to conducting the nursing assessment of the medical and psychiatric status of the consumers, Provide support as a liasion to community based medical providers regarding illness, medication, symptom management and activities and interventions to enhance well-being.
*********The location of this postition is Durham, NC. The Nurse will be scheduled to work 20 hours or less a week.***********
Salary: $27.90 per hour
Responsibilities/Duties
- Assist with monitoring for changes in emotional and physical health
- Review Medications as appropriate
- Ensure appropriate medications are available and documented accurately
- Develop health related goals in collaboration with the consumer and CTRP Clinician for service plan
- Assist in obtaining and/or interpreting medical information to improve the team member's knowledge base regarding consumers' health status and needs
- Consults with team regarding high risk behaviors exhibited by individuals served that could impact health status
- Participates in specialty trainings for team members and individuals receiving service
- Document services provided via electronic health record system
Required Experience
2 years' experience working in a medical facility or mental health setting is preferred.
Understanding of and interest in working with persons with serious mental illness in the community and home environment.
Required Education
ASN or BSN from an accredited nursing school with a current license to practice in the state of North CarolinaRequired Qualifications
Valid Driver's LicenseMust pass criminal background check
Program Summary
RHD North Carolina's Community Transitional Recovery Program (CTRP) provides short term increased supervision and support to individuals who have high and complex needs exiting state institutions or hospitals to further assess and prepare them for community-based living. CTRP provides a less restrictive alternative to hospitalization that further promotes recoveryand community integration. CTRP creates a continuum of care from crisis to community living with links to community services and supports.
