Job Description

For 50 years, Neighborcare Health has been caring for our neighbors with essential medical and dental care, counseling, outreach, health education and more in Seattle, Vashon, and our newest dental clinic in Olympia! The Housing Health Outreach Team (HHOT) is an RN-led multidisciplinary team providing services to formerly homeless individuals living in permanent supportive housing. HHOT uses a trauma-informed approach, harm reduction, and motivational interviewing to elicit patients’ health care goals, and support them in reaching them. RNs do active outreach to individuals within permanent supportive housing sites who experience significant barriers accessing primary care. The HHOT RN provides ongoing outreach and engagement whether residents identify a medical need or not, RNs offer patients a trusting relationship with a health care provider that will hopefully facilitate access to primary/specialty care when the patient is ready for that next step in care. The RN follows the Nursing Process: to assess, plan, implement and evaluate nursing care. RNs work in partnership with mental health, chemical dependency, case management, and other care team members to support patients in maintaining their health and housing.

Core responsibilities:

  • Conduct outreach within the housing site and support health care access for residents in permanent supportive housing.
  • Respond to emergent health issues
  • Work in partnership with care team members to plan and communicate care delivery.
  • Identify patient goals for improving their health status
  • Assess patient understanding of their disease and readiness to change using motivational interviewing and trauma informed
  • techniques.
  • Provide patient with education about the disease process and offer tools and skills needed to achieve self-management goals
  • including harm reduction.
  • Coordinate care with outside provider(s) and resources
  • Establish partnerships with patients, housing staff, friends, and families where appropriate, for positive outcomes in health status
  • Monitor, track, follow-up with patient
  • Follow protocol of best clinical practices to advise patients
  • Educate patients in the use of medications
  • Provide patients information to assist with self-care
  • Promote wellness and the importance of preventive care
  • Provide motivational interviewing and harm reduction counseling
  • Provide complex wound care (opportunities for training and support are available)
  • Perform and assist with procedures as appropriate by licensure and NeighborCare protocol and policy

Minimum Qualifications
Education
Required: Completion of an accredited RN program
Preferred: BSN desirable

License/Registration/Certification
Required: Valid WA State Registered Nurse license; BLS certificate
Preferred: Board certification in applicable specialty areas
