Salaried position or Pay per visit position with full benefits available

Our rates for pay per visits with full benefits are:

• $100 SOC
• $65 Re-certification
• $50 Skilled visits
• .50 cents per mile paid from your home and back!

Salaried rate: $78,000.00 to $80,000.00

PRN available as well!

The HCP of Texas RN Case Manager is responsible for the admission, planning, intervention, and ongoing evaluation of patients in home care. RN Case Manager must have expert level clinical assessment skills and make independent decisions for the well being of the patient.

Knowledge of the OASIS and be able to complete home visits as assigned.

RN Case Manager will partner with an LVN and have an assigned territory within a manageable proximity.

Experience with the following is required.

• Wound Care
• Wound Vac
• PICC Lines
• Ostomies
• IV infusion
• Suprapubic Catheter
• PT/INR
• Pleurx Drain
• Elastometric/Dial-a-flo
• TPN
