Job Description
The Registered Nurse performs professional and skilled nursing to patients in the community setting, to promote, protect, and preserve physical and mental health in the community. Plans and provides direct care, makes referrals to providers, and evaluates services as part of a comprehensive Community Health Program.
This position operates under the MNA bargaining unit. This is a part-time position, assigned to 28 hours/week. Complete pay scale is $24.02-$32.04/hr. For a complete position description, click here.
The primary focus of this position will be Family Home Visiting. This opening is being posted in conjunction with exam #00461 Public Health Nurse-Family Home Visiting. Only one of these two positions will be filled.Essential FunctionsThis job description indicates the normal type and level of work expected of the incumbent. Incumbent may be asked to perform other duties as apparent or assigned.
1. Assesses and evaluates the health status of individuals and families, collects baseline information, develops a plan of care in consultation with other professionals, and coordinates services with other providers and professionals.
2. Provides counseling and teaching to promote the highest level of self-care and wellness possible.
3. May be assigned to work in one or more programs which include, but are not limited to: WIC Nurse, WIC Coordinator, Follow Along Program, Maternal Child Health, Breastfeeding Counselor, Child and Teen Program including providing check-ups for children, physical examinations, hearing and vision screening, developmental testing, nutritional and dental assessment, etc.; and other programs.
4. Documents care so that all time, costs, and services provided are recorded accurately, including daily statistical and time studies.
5. Provides skilled nursing care and delegated medical functions, including prescribed medical treatment, to individuals and families.
6. Identifies, observes, reports, and records clients' symptoms and response to treatment, and general condition of clients.
7. Teaches health education programs, health-related classes, and presentations on Community Health Department Programs and other subjects, to community groups and special groups as requested. Develops and/or obtains content, curriculum, and handout materials.
8. Coordinates, schedules, and staffs health screening clinics in the County. Provides counseling and referrals as needed.
9. Identifies the need for and gives medically indicated immunizations, Tuberculin Tests, treatments, and medications.
10. Determines eligibility for waivered services, and functions as case manager.
11. Conducts hearing and vision screening programs.
12. Responds to and assesses public health-related nuisance issues.
13. Performs other job-related duties as assigned.Minimum QualificationsTwo years of post-secondary degree or four year Bachelor's Degree in Nursing and a valid Minnesota Registered Nurse License. A valid MN Driver's License is required. Two years of experience in Nursing, with one year in a public health related field is preferred. Certifications in Early and Periodic Screening, Red Cross Instructors for CPR, First Aid, Breast Feeding Counselor, Parenting, and/or other certifications may be required. Must satisfy continuing education requirements to maintain valid certification(s).
