You Will Be Responsible For:
- Timely, accurate, and thorough completion of medical documentation
- Ability to give immunizations
- Appropriate documentation of patient information on intake (including identified standards for documentation – i.e. mammogram, colonoscopy, smoking, etc)
- Ordering of pertinent medical supplies (including pharmaceuticals) and keeping inventory stocked
- Ability to utilize computer/computer software for pertinent patient documentation, providing documentation to patient as needed (i.e. patient plan). This includes pertinent training as necessary from time to time
- Adhere to policies/procedures for patient medications and testing – refilling patient medications according to protocols (SMPG policies)
- Electronic communications with patients, when desired (in keeping with established policy standards)
- Review of patient information via huddles to comply with established PCMH measures, with appropriate follow-up and/or communication as necessary.
- Excellent communication (both written and verbal)
- Effectively coordinate patient care with pertinent referrals and/or follow-ups (within protocol/policy standards) and in keeping with PCMH established standards
- Meaningful use compliance to standards
- Timely management of patient needs/requests (including phone calls) in accordance both with policy and established PCMH standards
- Maintain pertinent logs of work performed as detailed by Clinic Manager/Director
- Adhere to clinic specific competencies (checklist)
- Lead clinical team in PCMH measures, as directed by Manager/Director
- Associate’s Degree in Nursing
- Bachelors Degree in Nursing required within 4 years of hire
- Kansas Registered Nurse License
- BCLS obtained through the American Heart Association
