Job Responsibilities
- Clinical Practice & Care Coordination-
- The Inova Registered Nurse provides knowledgeable and caring clinical practice and care coordination through an understanding of patient, family, nurse and healthcare delivery team.
- Consistently provides safe, therapeutic care in a holistic and systematic way. Incorporates differences into the provision of care.
- All patient care and interactions are patient and family centered. Integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum to include patient and family education.
- Performs a comprehensive assessment by interpreting multiple, sometimes conflicting data and synthesizing the interrelationships of the data.
- Formulates an individualized plan of care based on assessment findings, interpreting trends in patient populations to achieve best practice.
- Supports and empowers patient/family as they progress through the healthcare continuum from health promotion to end of life decisions.
- Advocates patient rights from the patient/ family perspective; provides counsels and acts as a resource to resolve issues.
- Anticipates ethical/ spiritual/ cultural needs and intervenes to maximize patient outcomes. Coordinates health care and alternate services for patient/ family transition planning.

Additional Requirements
Recent hospital experience and/or previous nursing practice experience
At least six months of Registered Nurse clinical experience in the acute care setting
BSN or ADN. If RN has an ADN, within 6 months of start date the RN must meet with their nurse leader and * Identify which Accredited School the employee plans to attend * Submit a review of transcript from the school indicating the required pre requisites and timeline for taking the courses * Provide a written plan with anticipated BSN completion date. * Complete BSN within 24 months of start date
