Job Description
Harbor is seeking a Regional Care Coordination Director to provide leadership, direction and oversight for Care Coordination services within assigned catchment area(s). The Regional Care Coordination Director must hold an active clinical license (LISW-S, or LPCC-S) and will be responsible for working with local stakeholders to develop needed services and supports within the communities served. Ensures services adhere to High Fidelity Wraparound Principles and Trauma Informed Care model.
Education/Experience/Other Requirements:
- Master degree in mental health field, experience in Management or Clinical Supervision.
- Active Ohio LISW-S or LPCC-S.
- Must have or obtain within 90 days of employment an Ohio independent license. Must have and/or obtain within 15 months of employment licensure with supervisory endorsement.
- At least 3 years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or related field providing community-based services to youth and their families or caregivers.
- Must have experiences in at least one of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
- Culturally competent or responsive with training and experience necessary to manage complex cases
- Qualifications and experience to work with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders and who are engaged with one or more child-service systems.
- Prior Wraparound or Intensive Home Based Treatment experience preferred.
- Prior clinical and administrative management experience.
- Must be a team player, honest, self-motivated and organized.
- Excellent leadership and communication skills required.
- Able to effectively supervise, provide care coordination services and motivate staff to achieve maximum results.
- Demonstrated ability in developing and maintaining collaborative community relationships.
- Able to work successfully with interdisciplinary teams.
- Must have valid driver’s license, acceptable driving record, and be deemed insurable under Harbor’s vehicle insurance carrier.
- Must be proficient and accurate in computer use, including Microsoft Word.
- Required to complete High Fidelity Wraparound training and successfully complete skill and competency-based training to supervise delivery of ICC and MCC. Must obtain Health Officer certification within 6 months of employment.
Essential Job Competencies/Primary Duties:

- Ensures coverage of services, supervision, and crisis support across care coordination teams.
- Identifies formal and informal resources within the community and prepares annual resource development plan.
- Responsible for development and continued maintenance of community resource database.
- Reviews and analyzes feedback from youth and family/caregivers served, Child Family Team members, and program measures and implements programmatic changes to improve as necessary.
- Provides personnel management functions for all assigned staff including supervision, annual evaluations, involvement in hiring, disciplinary actions, etc. Any disciplinary action involving time off without pay or termination requires coordination/consultation with Human Resources.
- Assists in innovation, development and implementation and evaluation of services, including outcome and grant reporting
- Actively participates in community meetings and workgroups focused on enhancing services and community capacity to effectively serve youth and families.
- Provides care coordination services as needed.
- Provides leadership and supervision to support the team in fulfilling or exceeding program contractual requirements.
- Monitors and manages team budget.
- Maintains client records and completes all documentation on a “real-time” basis.
- Assures that all clinical files of the team meet all certification and accreditation requirements.
- Reviews clinical documentation and co-signs as required.
- Assists with preparing the team for accreditation, licensure, certification and audits.
- Ensures customer satisfaction for both internal and external customers.
- Provides consultation and education services to individuals and groups in the community as needed.
- Keeps current with trends and developments related to essential job competencies, and demonstrates continued growth.
- Reviews “Authorizations to Disclose / Obtain Confidential Information” (ROIs/PHIs) as. requested by Medical Records for ROIs needing clinical review.
- Participates in external committees, events, and media coverage to ensure Harbor representation.
