Access: Supports For Living is seeking a Caseworker for our Real Changes program. The Caseworker provides intensive casework counseling to Juvenile delinquent and PINS related youth ages 10-18 with emotional, social and educational needs/deficits who are at risk of being placed outside of the home. Issues that are addressed include, but are not limited to, community behavior, mental health needs, school truancy and academic performance, home behavior and independent living skills. Caseworker position requires intensive contact with community resources and other service providers to ensure and encourage youth and families to make positive changes in overall functioning ability and prevent out of home placement. Must be comfortable addressing situational issues and make court appearances. Transporting clients using your own, reliable vehicle is also required.



Job Description:

In this role you will:

  • Provide intensive casework counseling to Juvenile Delinquent and PINS related youth.
  • Maintain intensive contact with community resources and other service providers to ensure and encourage youth and families to make positive changes in overall functioning ability and prevent out of home placement.
  • have flexibility to accommodate clients, which includes before/ after hours and weekend work when necessary. Position requires 24 hour on call duties for crisis intervention when needed.
  • Caseworker will be responsible for reporting and documentation of all contact with clients and collaterals, as well as intensive face to face contact with youth and families.

Qualifications:

The ideal candidate will have:

  • One year of related Casework experience preferred
  • BA Degree in Human Services or related field of study
  • Excellent Verbal and written communication skills
  • Organization
  • Excellent computer skills
  • Flexibility a MUST
  • Bilingual Spanish/English a plus
  • Familiarity with NYS Connections system is preferred.
  • Must have own, reliable vehicle and valid driver's license.
