Job Description

JOB SUMMARY:

The Qualified Mental Health Professional Lead – Transitions implements and oversees individualized social and living skills development program and provides direct care including supportive level counseling and skill training for individuals who have serious and persistent mental illness and live in a supervised residential facility on a 24/7 basis. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, public and private payor guidelines, professional practices and including ethical standards related to client interactions, state and federal licensure and accreditation regulations and guidelines.

ESSENTIAL FUNCTIONS:

  • Complete administrative duties to include but not limited to: Leading the day-to-day operations of the transitional home, including coordination of clinical services, lead transitional home meetings, evaluate the status of individuals and facilitate appropriate planning and coordination of treatment activities to ensure immediate attention to changing needs, assist program employees and administration to create and maintain a therapeutic milieu; attend administrative meetings relevant to the transitional home program and its' identified goals, provide monthly supervision to direct supervisees and participate in monthly supervision with supervisor, develop and maintain program policies and procedures that continue to adequately describe the intended function of the transitional home, meet its' intended goals, and ensure a framework of practice that can be implemented and clearly followed by transitional home employees.
  • Facilitate referral/admission process to transitional home to ensure maximum utilization of beds possible in accordance with the expectations of MRCSB and DBHDS.
  • Work in coordination with psychiatrist/NP as well as outpatient clinical staff to ensure that individuals' identified treatment needs are adequately addressed.
  • Provide supportive counseling to individuals as well as training and support as needed to program employees and assist in monitoring of Individual Service Plans, implements specified services, prepares quarterly updates to the ISPs.
  • Assists in the upkeep of the house, grounds, and vehicles while ensuring that all inspections are completed as necessary including but not limited to: Fire drills, smoke detectors, fire extinguishers, etc. and maintains appropriate records.

QUALIFICATIONS:

  • Good oral and written communication skills.
  • Knowledge of mental health, substance abuse, and ID symptomatology.
  • Knowledge of treatment modalities and case management approaches, such as service coordination, behavior management, independent living skills training, supportive counseling, family education, and discharge planning.
  • Knowledge of crisis intervention strategies and procedures.
  • Ability to implement and lead individual and group training sessions, focusing on increasing recovery and self-determination.
  • Knowledge of mental health, ID, and substance abuse confidentiality guidelines
  • Valid Virginia Driver’s License without any points
  • Upon hire, individual will need to complete CPR/First Aid Training, Medication Administration training, and Safety Care.

Minimum Requirements
  • Bachelor's degree in a Human Services field required. Knowledge of mental illness/substance abuse symptomology and experience working with individuals with mental health and substance abuse disabilities.
  • Must meet Board of Health Professions registration requirements for QMHP (A or C or A-Trainee or C-Trainee).
  • Three years’ experience working with individuals with serious and persistent mental health needs.
