Job Description

Job Title: Part-time Community Health Outreach Worker

Job Summary:

BOOM!Health is seeking a Community Health Outreach Worker

Conduct staff-led outreach, engagement/re-engagement strategies including accompaniment, client assistance, follow up calls and visits, specifically designed to retain participants in care and support services. Provide outreach, engagement, education and referrals to individuals at a variety of venues including Single Room Occupancy (SRO) hotels, drug encampments, parks, street outreach in the North and South Bronx.

Essential Duties and Responsibilities include, but are not limited to;

· Initiate identification and enrollment of eligible clients to agency services and programs.

Day and evening comprehensive outreach* at targeted SRO hotels, including engagement and distribution of prevention and reduction supplies (*includes climbing 4-5 flights of stairs SRO hotel sites) drug encampments, parks, street outreach in the North and South Bronx. Assist with syringe litter cleanups in the community

· Syringe exchange and related education at identified SRO hotels

· Preparation and packaging of outreach supplies

· Education and information for harm reduction, safer sex, infectious disease transmission and prevention

· Information for housing, services and care, including referrals for primary care, mental wellness, care management, nutritional services, and related needs

· Arrange and provide transportation services

· Provide advocacy for participants with providers, as needed

· Conduct program enrollments for new participants

· Documentation of all activities and services provided

· Participate in Case Conference, team meeting, assigned trainings, and individual supervision sessions

· Develop and implement appropriate outreach strategies and modalities per SRO hotel, in collaboration with SHOP Coordinator

· Representation of program and participant needs at various community forums and planning bodies, as assigned

· Other related duties, as assigned

Qualifications/Criteria :

· Experience in harm reduction theory and practice

· Knowledge and experience with outreach strategies and provision

· Minimum one year experience working with active drug users and homeless in a community-based setting

· Experience working with people living with HIV/AIDS

· Knowledge and experience in disease prevention and health education

· Excellent people skills and oral communication skills

· Flexible team player

· Knowledge of social services in NYC

· Clean, valid driver’s NYS license with at least 3 years driving experience

· Bilingual English/Spanish preferred

· Experience with AIRS and/or eSHARE reporting systems (training provided)

· Willing to work flexible hours, including some evenings, weekends and late nights.
