Psychosocial Rehabilitation Specialist serves children and adults diagnosed with severe and persistent mental illness. As a PSR Specialist, you are required to prepare and execute group and individual interventions to assist clients in restoring a functional deficit in the areas of independent, social, and vocational skills. Help clients adjust to their social environment. May work with individual and group counseling and provide assistance with job training, housing needs, and family support issues. PSR assesses the client’s current conditions and level of social functionality. Develop a treatment plan to help clients learn skill sets needed for independent living, as well as maintain daily living skills. PSR monitors client progress and helps to achieve their social integration goals. Manage client records to ensure proper clinical documentation is maintained and meets Medicaid standards. Other duties as assigned by supervisor and adhere to all policies and procedures of Angels For Kids On-Call.
RESPONSIBILITY
Plan, prepare and execute group and individual interventions to assist the client in reaching their therapeutic goals
- Provided psychoeducation, psychosocial rehabilitation training in the areas of basic living skills development, psychosocial skills training, therapeutic socialization, and client empowerment.
- Provide psychosocial rehabilitation activities in either a group or individual setting. These services must be provided in a facility-based setting, either in an integrated program model (rehabilitation training or a structured group on-site).
Individual services should be provided on a one to one private setting.
- Develop treatment plans addressing the specific needs of the clients with individualized and measurable goals for treatment.
- Maintain confidentiality of all clients.
- Complete the necessary documentation in the required time frame regarding the client’s mental status, responses to interventions, and progress toward treatment goals.
- Manage client records to ensure proper documentation is maintained and meets applicable standards.
- Adhere to policies and procedures of Angels for Kids On-Call.
- Other duties as assigned by the supervisor.
