I) Job Purpose
A) The Continuity of Care Liaison will work with the Probate Courts, District Attorney’s Office, local hospitals and state hospitals to ensure they receive timely and accurate information about our clients to help determine the best course of treatment for the safety of the client and the community.
II) Essential Functions
A) Provides the Tarrant County Probate Courts with an Alternative Treatment Recommendation (ATR)
for each person recommended to the Court for Temporary Commitment, as a representative of the
Mental Health Authority, in accordance with the Texas Mental Health Code Standard:
i) Standard:
(a) Obtains daily filings from the County Clerk
(b) Cross checks court filings with hospital case submission lists to make sure all clients are accounted for
(c) Reviews all filings and determines if they are in the CMHC/Avatar and the CARE system
(d) Places all filings on a court docket and distributes cases evenly amongst staff
(e) Cross checks court filings with hospital lists to make sure all clients are accounted for
(f) Develops written ATR (long-form for JPS Hospital and short-form for private hospitals) to meet documentation standards of the Probate Courts
(g) Meet with the treatment teams face-to-face or contact them by phone to determine the least restrictive alternative placement setting for all persons on the court docket
(h) Scans and emails all ATRs to the District Attorney’s Office, the Attorneys Ad Litem and the treatment teams no later than 3:00pm the day prior to court
(i) Attends commitment hearings and testifies as called upon by Judge
(j) Completes disposition column of the docket and distributes it to the Court/Aftercare Team and the JPS Treatment Teams
B) Functions as part of the Court and Aftercare Team
i) Standard
(a) Acts as liaison and maintains flow of information between MHMR Tarrant outpatient clinics and programs and JPS inpatient services.
(b) Provides the PEC physicians and social workers with diagnostic medical history on MHMR Tarrant clients who present to the PEC
(c) Relays all pertinent information on MHMR Tarrant clients who present in the PEC to appropriate MHMR Tarrant staff.
(d) Acts as a resource coordinator by maintaining up-to-date resource referrals and providing said resources to PEC staff and MHMR Tarrant clients.
(e) Assists in PEC record keeping functions.
(f) Completes continuity of care service activities for MHMR Tarrant clients, including scheduling follow-up appointment with for client with the clinic and provided clinic with admission and discharge information.
(g) Provides face-to-face consultation and participates in difficult case consultations as needed.
(h) Completes monthly report of all active MHMR clients who presented to the PEC, complete with disposition

D) Accurately handles the Mental Health Warrant (MHW) Process
i) Standard:
(a) Determines whether MHW request is valid and is able to defer when appropriate by providing applicant with community resources.
(b) Accurately completes the MHW letter and communicates with the appropriate Judge regarding the specific information provided by the applicant without errors.
E) Ensures the continued integrity of Continuity of Care job functions.
i) Standard:
(a) Assists with training of new and existing staff regarding the functions and process of the Continuity of Care Department as requested.
F) Maintains proficiency in computer software applications specific to the Continuity of Care job functions
i) Standard:
(a) Maintain expertise in CARE, myAvatar, EPIC and other programs required to research client information.

III) Knowledge of Laws, Regulations, Policies/Procedures, Skills, and Abilities

IV)Internal & External Customer Service
Extensive internal and external contact is required. The employee will demonstrate advanced written and verbal skills while maintaining a positive image of the program and agency.
V) Travel
- This position requires travel 10% of the time in personal vehicles.
Required Education
High School Diploma

Required Experience
Four (4) years experience
Substitutions
Bachelor’s Degree from an accredited college or university with major course work in Social Work, Behavioral, or Human Services. Administrative Support/Clerical.
Required License(s)
Valid Texas driver’s license with an acceptable driving record.
Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate
