The Project Manager will be responsible for leading, coordinating, monitoring and maintaining the projects for the development and growth of care coordination throughout the health system any other assigned special projects. Incumbent will provide delegation oversight, implementation support, measurement standards, and program revisions, as needed that incorporate applicable best practices. Plan strategically to reach long-term goals of each project, as well as solve problems that may impede reaching set goals. Develop care coordination redesign, training programs, onboarding structure, collaborate and manage projects with the inpatient and hospitalist teams to align our efforts. Incumbent must demonstrate managerial and leadership skills to support administrative and professional staff in ambulatory care coordination. Report to the Director of Ambulatory Care Coordination and Perform most of the assigned duties with minimum supervision; work independently and prioritize own work assignments for majority of responsibilities. Where necessary, seek advice of supervisor.
QualificationsA minimum of three years recent experience within a healthcare setting in a project team role, with a proven history of high performance.
LEAN certification preferred.
Strong project management skills with the proven ability to deliver agreed objectives with timeframes, including experience in project outcome reporting in a user friendly format.
Experience in leading, facilitating, and implementing new programs and/or projects.
Proven history of ability to successfully relate and communicate with all levels of staff from different professional backgrounds.
Skill in analyzing information, problems, situations and procedures to define objectives, identify pattern and formulate conclusions.
Ability to creatively integrate competing demands of an outpatient setting into a productive working environment.
Ability to interact diplomatically and sympathetically with a patient population in varying degrees of health and effective communication with staff, faculty and the general public in conveying information to varying organizational levels.
Interpersonal skills and ability to communicate effectively and diplomatically with patients, staff and physicians; give and receive information, clarify policies, and mediate if necessary with patients, physicians, and technologists.
Ability to accept and carry out oral instructions accurately and work independently as well as a part of a team within a demanding environment.
Ability to prioritize work, handle multiple tasks to completion and make decisions on which course to follow for a particular situation.
Ability to work independently and follow through on assignments with minimal direction and supervision.
Knowledge and familiarity with the Electronic Health Records, (EPIC) preferred.
Ability to be flexible with working hours given adequate notice and in alignment with job demands.
Ability to travel to multiple clinics.
Knowledge of computer software application including Microsoft Office (Word, Excel, and Power Point)
