Position: Post-Partum Program Coordinator OB/GYN

Department: DOB OB/GYN Support Staff

Schedule: Full Time

POSITION SUMMARY:

This is the key administrative position for Post-Partum Care Coordination, for the department of Obstetrics and Gynecology at Boston Medical Center; the largest safety net hospital in Boston. The Post-Partum Program Coordinator will be responsible for improving our patients' access to and uptake of post-partum care. The post-partum program coordinator will assist in the design of performance objectives and implement and monitor programming to meet desired outcomes. Along with the medical director, operations managers, and prenatal team; the program coordinator will plan to improve health outcomes by increasing engagement with post-partum care and helping mothers transition into primary care.

Responsibilities Include:
  • Improve attendance of postpartum visits, both telehealth and in-person visits
  • Increase completion rate of post-partum depression screening and ensure connection to psychiatry and/or social work for positive depression screens
  • Continuously monitor our panel of families who get their pediatric and obstetrics care at BMC and identify any mothers who are due for post-partum visits and have not completed appointments.
  • Increase completion of 2hr glucose tolerance tests for women with GDM
  • Ensure connection and follow through to primary care in the first year postpartum for women with high risk comorbidities:
    • Hypertension
    • Diabetes type 1 or 2
    • Women w severe preterm preeclampsia
  • Manages Master List of Post-partum No-Shows
  • Connect post-partum women with PCP
  • Connect Post-partum with contraception needs
  • Connect people to resources around breastfeeding
  • Ensures post-partum patients are scheduled with their continuity provider or preferred provider scheduling
  • Trains New Hires on postpartum scheduling
  • Organize training seminars to address staff education related to postpartum care coordination
  • Meet with women on PP before they are discharged to make sure their appts are coordinated between pedi and OB as much as possible
  • Responsible for systemic improvement of referral coordination amongst the different services
  • COVID care coordination and follow up
  • Serves as liaison between community health centers and BMC
  • Organizes and establishes meetings on regular basis with designated individuals to foster positive relationships
  • Ensures positive patient and provider experience
  • Serves as liaison between community health centers and BMC
  • Assists in the development of short and long term plans for programs, monitor progress, assure adherence and evaluate the performance
  • Supports the medical director in the design of performance objectives and implements and monitors programming to meet desired outcomes

EDUCATION:
  • Bachelor's Degree (or equivalent experience) required, preferably in healthcare, or related field.
  • Minimum of 3 years relevant experience required. Non-profit program coordination is a plus.
  • Knowledge of program evaluation is helpful.
