POD Social Worker

Job Summary:

The Social Worker provides a variety of client-centered clinical interventions and clinical support activities, including conducting biopsychosocial intakes, care plan development, implementation, evaluation, and modification, supportive counseling, crisis intervention, assessment and referral to support services, and clinical consultation. Services are provided through face to face, phone, and other contacts

The Social Worker provides the opportunity to act as a Field Instructor for the MSW program. Conduct comprehensive biopsychosocial assessments for a diverse patient population. Complete psychosocial section of initial assessments within 30 days of assignment and conduct mandated outreach to facilitate completion of the initial assessment. Develop, coordinate, and implement a client-centered multidisciplinary care plan that establishes SMART goals and includes risk reduction strategies as needed.

Education & Experiences:

• Master’s degree in Social Work
• LMSW required, Licensed Clinical Social Worker in the state preferred
• 5 years experiences with patients who have multiple chronic medical conditions, and acute/chronic behavioral health needs
• Knowledge of managed care and insurance processes is preferred.
• Experience with the management of Medicare, Medicaid, and commercial populations.
