The Palliative Care Program is one of the many VA NJ programs that is under Geriatric Extended Care (GEC).
S/he reports to the Associate Chief of Staff, Extended Care Service or designee. S/he will be part of the Palliative Care Consult Team and will be serving Veterans inpatient East Orange, Home Based Primary Care, Outpatient PACT, CBOC, and subspecialty services.
S/he will provide consultation for hospice and palliative care for Veterans who are under home based primary care, inpatient, outpatient (CBOC, PACT), VA Community Living Center (CLC). These services include but not limited to advanced care planning, symptom management, inpatient HPC services, collaboration and case findings with PACT/CBOC interdisciplinary team and with community hospice partners. S/he will do the consultation face to face, Video VA connect, telephone, and electronic consults.
- Performs all Palliative care consults to Veterans needing Palliative care in outpatient (CBOC, PACT, HBPC) and inpatient East Orange.
- Provides coverage in Palliative Care where it is needed.
- Provides and implements action plans on national performance measures
- S/he will work closely with teams in PACT, CBOC, HBPC and inpatient EO to deliver Palliative care service
- S/he will participate in committees such as Mortality and Morbidity, Cancer Committee and share information to Palliative care team
- S/he will rounds and participate in huddles with PACT and subspecialties, CBOC, HBPC for case findings
- Collaborating with other services, VISN leadership, and VA Central Office staff on issues of program development and operations.
- Processing, assigning, and monitoring referrals of Palliative Care to ensure timely access and care coordination.
- Provides clinical leadership within the Extended Care Service and collaborate with the Supervisors of the service in the identification of focus areas for performance improvement which includes:
- trends in patient care, cluster activity, and specific areas impacting patient safety.
- Feedback from customer satisfaction measures and factors contributing to high customer satisfaction.
- Factors contributing to staff satisfaction and retention.
- Participates with national Palliative care and VISN 2 conference calls/meetings
S/he will complete all consults inoutpatient PACT, CBOC, HBPC, and inpatient. The goal is to increase access by targeting Veterans in the outpatient area. The workload also includes educational/informative outreach to PACT/CBOC and HBPC teams in Lyons and East Orange related to VA NJ Palliative Care Program. S/he will work with PACT/CBOC teams and would be the POC and be readily available to provide PACT staff education about palliative care and comfort care. The Physician would also do Resident educational series in East Orange for throughout the services, and will attend monthly resident orientation to keep orienting them to timely referral, timely intervention, and what is an appropriate referral. The Palliative Care Physician will also do case finding by identifying high CAN scores amongst the Veterans under Home Base Primary Care, CBOC, PACT and will include outreach to community groups. The Palliative Care Physician will work closely with and learn from the existing inpatient VANJHCS' palliative care consult team and outpatient palliative care clinic as well as the VA NJHCS Home Based Primary Care (HBPC) that also includes Medical Foster Home and Rural Health.
Travel: 50% or more
Work Schedule: 8AM to 4:30PM Monday thru Friday
