We are seeking a Physical Therapy Assistant - Home Health East Portland that will provide skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities, taking into account individual patients special physical or age-related needs. Requires skills in patient/client treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with individual's functional level and potential.
In this position you will have the following responsibilities:
- Treat patients to relieve pain, develop and restore function and maintain maximum performance using physical means, such as exercise, massage, heat, cold, ultrasound and electrical stimulation
- Direct and aids the patient in active and passive exercise, re-education, gait training and functional training and activities of daily living including transfer and prosthetic training
- Report observation of patients/clients reaction to treatment to the physical therapist
- Manage a caseload as assigned by the physical therapist
- Manage equipment safety and adapts, fits and instructs in the care and use of equipment to the patient/client
- Teach/instruct patient/client and caregivers to assist patient/client in achieving treatment goals
- Confer with physical therapists, physicians, families, and other caregivers regarding treatment plans, expected results and goals
- Record clinical, progress notes and itinerary records to meet department standards, State and Federal regulations in a timely and accurate manner
- Terminate patient/client treatment, upon approval of physical therapist, based upon condition/diagnosis and functional performance outcome expectations
Qualifications:
Required qualifications for this position includes:
- Associate's Degree in Physical Therapy, from an accredited program
- Physical Therapy Assistant licensure through the State of Oregon
- Valid driver's license
- Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
- Current BLS (Basic Life Support for Health Care Provider), -OR-
- ACLS (Advanced Cardiac Life Support) required within 30 days of hire
- Staff that provide service in the State of Washington require a Washington State physical therapy assistant license
