Job Description

Position Goal

Staff Pharmacist is responsible for accurately monitoring patient medication profiles, input and filling of medication orders, providing drug information, and supervising pharmacy technicians. Ensures adequate control and documentation of all controlled substances and records. Ensures appropriate and conservative use of antibiotics throughout facility. Assumes the responsibilities and performs the duties of the director of pharmacy in their absence.

Position Responsibilities:

  • Ensures that formulary medications are available for immediate use within the hospital
  • Ensures that medications are stored under proper conditions of sanitation, temperature, light, moisture, ventilation, and segregation
  • Maintains the safety and security of all medications
  • Ensures that emergency carts, medications, and supplies are consistently available, controlled, and secure throughout hospital
  • Monitors drug therapy regimens for contraindications, therapeutic duplications, drug-drug interactions, drug-food interactions, drug-disease interactions, allergies, renal status of the patient, and appropriateness of drug and dose.
  • Provides accurate, adequate, and timely drug information to the professional staff
  • Ensures that medications are prepared safely and under proper conditions
  • Ensures that medications are appropriately labeled and expiration date noted
  • Ensures that any medications are dispensed and distributed safely and accurately to the patient for whom they are ordered
  • Suggests appropriate, cost-effective therapeutic alternatives to staff, as needed
  • Maintains adequate stock of medications and supplies according to established policies and procedures while making cost conscious decisions
  • Supervises the ordering, receiving, unpacking, and storage of pharmaceuticals and supplies in appropriate locations throughout the facility
  • Ensures stock is rotated and used before expiration
  • Ensures that outdated drugs are identified, quarantined, and replaced
  • Ensures that all invoices are reviewed, recorded, and reconciled
  • Provides appropriate leadership that promotes the safe, effective operation of the department in the absence of the director
  • Daily monitoring of antibiotic usage for all patients through review of cultures, antibiograms, and orders making recommendations to prescribers that will most appropriately and conservatively treat patients on all antibiotics
  • Monthly participation in the AHRQ review of antibiotic stewardship program throughout facility to monitor and improve antibiotic use and prescribing habits throughout the facility
  • Leading/participation in antibiotic stewardship activities and initiatives of the facility
  • Inputs written and electronic medication orders and reconciles retrospectively patient medication profiles accurately and in a timely manner
  • Supervises and directs pharmacy support personnel, pharmacy technicians, and other departments throughout the facility
  • Organizes and prioritizes work assignments
  • Maintains logs, records, and other required documentation accurately. Files documents in appropriate locations
  • Demonstrates good oral and written communication
  • Maintains accountability for all controlled substances and controlled substance record keeping
  • Participates in departmental programs that improve the quality of pharmacy services provided
  • Ensures completion and documentation of all medication storage area inspections monthly. Follow up and correct any identified deficiencies
  • Attends required departmental and facility in-services

Experience: 1 year hospital experience preferred.

Education: Bachelor of Science in Pharmacy required, Doctor of Pharmacy preferred

Special Qualifications:

  • Current pharmacist registration with the Alabama State Board of Pharmacy,
  • BLS required
